Iatrogenic renal vascular injury: Diagnosis with 3-D CT angiography and endovascular repair

Özet

Günümüzde nefrolitiyazis tedavisinde perkütan nefrolitotomi (PNL), minimal invaziv bir yöntem olup açık cerrahiye alternatif bir tedavi yöntemi olarak yerini almıştır. PNL’ye bağlı arteriovenöz fistül, pseudoanevrizma gibi çeşitli renovasküler yaralanmalar oluşabilir ve sıklıkla hematüri ve/veya retroperitoneal hematom ile bulgu verir. Bu istenmeyen yan etkiler çoğunlukla koruyucu tedaviye cevap vermekle birlikte, bir grup hastada ileri tetkik ve kısmi hatta total nefrektomiye kadar varan saldırgan tedaviler gerektirebilir. Ultrason bu hasta grubunda, tanı için ilk başvurulan görüntüleme yöntemi olup özellikle retroperitoneal hematom açısından değerlendirme sağlar. Kateter anjiyografi tanıda altın standart yöntem olarak kabul edilmektedir ve gerekirse aynı seansta selektif böbrek embolizasyonu seçeneği de en sık kullanılan tedavi yöntemidir. Biz burada, perkütan nefrolitotomi sonrası perzistan hematürisi olan bir hastada, 3 boyutlu bilgisayarlı tomografi anjiyografi tetkiki ile intrarenal psödoanevrizmaları tespit edildikten sonra, mikrokoil ve nBCA kullanılarak endovasküler yolla tedavisini sunuyoruz.

Abstract

Introduction: Currently, percutaneous nephrolithotomy has been well established procedure as a minimal invasive treatment alternative of open surgery in the treatment of nephrolithiasis. Several renal vascular injuries can happen during the treatment such as arteriovenous fistula and pseudo aneurysms and frequently present as hematuria and/or retroperitoneal hemorrhage. Although these complications can generally be managed by conservative approaches; more aggressive treatment options (even total nephrectomy) may be needed in some of these patients. Ultrasound is the first line imaging modality in recognition of these complications particularly for detection of retroperitoneal hemorrhage. The gold standard diagnostic modality is catheter angiography and possibility of having a selective transarterial embolization at the same session is the leading treatment alternative for these complications. Herein, we present a case of renal arterial injury related to percutaneous nephrolithotomy who present with persistant hematuria. The diagnosis of intrarenal multiple pseudoaneurysms was made with 3-dimensional CT angiography and the treatments was done subsequently using microcoils and nBCA via transarterial approach.
