Özet

Göğüs duvarı defektleri sıkça karşılaşılmaya başlanan ve onarımları ile iyi fonksiyonel ve kozmetik sonuçların elde edilmesi mümkün olan defektlerdir. İyi bir rekonstrüksiyon uygun ve yeterli şekilde sağlıklı dokulara kadar rezeksiyon yapmayı, ölü boşluk ve kaviteleri iyi vaskülarize dokularla doldurmayı, eğer gerekli ise göğüs duvarı iskelet stabilitesini sağlamayı ve yumuşak doku örtüsünün sağlanmasını içerir. Kliniğimizde 4 bayan 2 erkek altı hastaya tam kat göğüs duvarı rezeksiyonu yapıldı. Hastaların yaşları 37 ila 77 arasında değişmekteydi. Hastaların takip süreleri 12 gün ile 18 ay arasında değişmekteydi. Üç vaka meme karsinomu nüksü, 2 vaka sarkom nüksü, l vaka ise radyasyona bağlı göğüs duvarı lezyonu olan hastalardı. Yumuşak doku örtümü için lokal deri, kas, kas deri flepleri ve serbest latissimus dorsiflebi kullanıldı. İskelet stabilizasyonu için fasya ve marleks mesh-metilmetakrilat çok katlı protez kullanıldı. İki hasta biri erken olmak üzere .postoperatif dönemde kaybedildi. Diğer dört hastada stabil iskelet ve yumuşak doku örtümü sağlandı. Donor ve alıcı alanlarda sutür hattında ayrılma, hematom, sutür hattında minör enfeksiyon, marjinal nekroz, serama, greft kaybı karşılaştığımız komplikasy onlardı. Üç ve daha fazla kosta rezeksiy'onlarında marleks meshmetilmetakrilat çok katlı protezler ile iskelet stabilizasyonunun sağlanması ve yumuşak doku örtümü için öncelikle lokal kas-derifleplerinin kulanılmasının etkin bir tedavi yaklaşımı olacağı kanaatindeyiz.

Abstract

Chest wall defects are frequently encountered diseases that good results can be obtained both cosmetically and functionally. Satisfactory reconstruction defines an appropriate and sufficient resection to the point of reaching the healthy tissues, the filling of the dead spaces and cavities wiih well vascularized tissues, and if required teh provision of the stability of the chest wall skeleton and coverage with soft tissue. Six cases, 4 females and 2 males, have been treated withfull thickness chest wall resection at our clinic. The ages of the patients varied between 37 and 77. a follow up period changing from 12 days to 18 months was determined. Three of-that patients had suffered from recurrent breast carsinoma, two from sarcoma relapse and one from chest wall lesion due to irradiation. Local skin, musculocutaneous and free latissimus dorsi flaps have served as sofi tissue coverage. Fascia and "Marlex " mesh-methylmethacrylate sandwich prosthesis were employed for skeletal stabilization. Two of the patients have deceased in the postoperative period, one of which early. With the other 4 patients, stable skeleteon and sofi tissue coverage has been achieved. Detachment and minör infection in the suture lines, hematoma marginal necrosis, serama and graft loss in the donor an recipient sites vtere the complications that we have come across with. Based on our observation the used of "Marley," mesh-methylmethacrylate sandwich prosthesis for the skeletal stabilization and primary use of local musculocutaneous flaps for soft tissue coverage in cases of resection of 3 or more ribs is an ejfective therapy approach.
