Özet
Mikrotiada kulak rekonstrüksiyonu zor bir işlemdir. Amaç, ince ve sağlıklı bir cilt ile kaplanmış, heliks ve antiheliksin kıvrımlarına sahip, karşı taraf kulak kepçesiyle simetrik ve aynı projeksiyona sahip bir dış kulak yapılmasıdır. Kulak iskeletini oluşturmak için, genellikle otojen kostal kıkırdak grefti kullanılmaktadır. Fakat, bu greftin şekillendirilmesi yetenek ve deneyim gerektirir ve alınması sırasında bazı morbiditelere sahiptir. Bu nedenle bazı alloplastik implantlar geliştirilmiştir ve bunlar arasında en uygun görüneni Medpor implantlardır. Kliniğimizde, 3 yıllık dönemde, 8 mikrotia olgusunda kulak rekonstrüksiyonu Medpor implant ve temporal fasya flebi kullanılarak yapılmış, 3 olguda ekspozisyonla karşılaşılırken 5 olguda tatminkar sonuçlar elde edilmiştir.
Abstract
Auricle reconstruction in microtia is a difficult process. The aim is building up an auricle which is symmetrical and has the same projection with the opposing one. It should also have convolutions of helix and antihelix and should be covered with a thin and healthy skin envelope. Otogenous costal cartilage graft is the usual choice for the framework of the new auricle, but sculpting the framework requires competence and experience and it has some donor site morbidities. For this reason, some alloplastic implants were developed and among these, Medpor seems to be the most suitable one. In three years period, 8 microtic ears were reconstructed by using Medpor implants and temporal fascial flap at our clinic. In 3 cases, we faced with exposition of the implant and in 5 cases we obtained satisfying results.
