Özet
Perforatör flepler 1980'li yıllarda popüler olmuş kas-deri ve fasya-deri fleplerinin geliştirilmesi sonucu ortaya çıkmıştır. Kas dokusunun korunmasıyla verici alan morbiditesinin ve fonksiyon kaybının azaltıldığı bu yöntem bir çok klinik durum için öncelikli rekonstrüksiyon yöntemi haline gelmiştir. Bu çalışmanın amacı, perforatörflep uygulamalarının gelişimini literatür bilgileri ışığında derlemek ve kliniğimizde gerçekleştirilen perforatör flep uygulamalarını tartışmaktır. Kliniğimizde, Kasım 2001-Temmuz 2003 tarihleri arasında 8 hastada, 10 adet serbest derin inferior epigastrik arter perforatör flebi ile meme rekonstrüksiyonu gerçekleştirildi. Hastalara uygulanan 10 flepten 7'sinde hiçbir sorun gözlenmedi. Bir hastada venöz yetmezlik nedeniyle flep kaybedildi. Birinde arteriyel trombus, diğerinde hematom sebebiyle dolaşım problemi gözlenen iki flep gerekli müdahalelerle kurtarıldı. Sadece deri-derialtı dokusuna ihtiyaç duyulan rekonstrüksiyonlarda sadece deri-derialtı dokusundan oluşan bir flebin kullanılması ve altındaki kasın bu flebin dolaşımı için feda edilmemesi oldukça mantıklı görünmektedir. Perforatörfleplerin sunduğu geniş alternatifler, özellikle meme rekonstrüksiyonu gibi üç boyutlu defektlerin rekonstrüksiyonunda ve flep kalınlığının dezavantaj oluşturabileceği sığ yaraların kapatılmasında bu flepleri ideal hale getirmektedir.
Abstract
Perforator flaps have evolved as ah improvement over the popular musculocutaneous flaps and fasciocutaneous flaps developed in the 1980s. By sparing muscle tissue, thus reducing donor site morbidity and functional loss, perforator flaps are indicated for a number of clinical problems. We reviewed the historical development of the perforator flaps and discussed our experience with these flaps. Ten free deep inferior epigastric artery perforator flaps in 8 patients were performed between November 2001 and July 2003. No problems were observed in 7 out of 10 flaps. One flap was lost due to venous insufficiency. Two flaps showing early circulatory problems caused by hematoma and arterial thrombosis in two cases were salvaged by appropriate management. When only,skin is needed for a specific reconstruction, it makes logical sense to transfer only skin to the recipient site while preserving the integrity of the muscle at the donor site. The versatility of the perforator flap makes it ideal for the reconstruction of three-dimensional defects such as breast reconstruction or as a thin flap for resurfacing shallow wounds when bulk is considered a disadvantage.
