Özet
Pediatrik yaş grubunda mandibula kırıkları nadir görülür ve erişkinlere göre farklı klinik özellikler gösterir. Etyoloji ve kırık patternindeki farklılıkları vurgulamak amacıyla, bu retrospektif çalışmada, Uludağ Üniversitesi Tıp Fakültesi Plastik Rekonstrüktif ve Estetik Cerrahi kliniğinde 1992-2003 yılları arasında tedavi edilen toplam 30 pediatrik mandibula kırığı olgusunun analizi sunuldu. Olgular, yaş, cinsiyet, etyolojik faktörler, kırık lokalizasyonu ve sayısı, eşlik eden yaralanmalar, uygulanan tedavi yöntemleri ve komplikasyonlar yönünden değerlendirildi. Hastaların 7'si kız (%23.3), 23'ü erkek (%76.6) olup, yaş ortalaması 6.1 yıl idi. Kırıkların oluş nedenleri, 13 olguda (%43.3) yüksekten düşme, 12 olguda (%40) motorlu araçlarla oluşan trafik kazası, 5 olguda (%16.6) bisiklet kazası idi. Sadece 16 olguda (%53.3) izole mandibula kırığı mevcut iken 14 olguda (%46.6) eşlik eden diğer yaralanmalar mevcuttu. Kırık en sık 12 olguda parasimfiz (%40) ve 11 olguda kondil (%36.6) bölgesinde görüldü. Sadece 18 olguda (%60) tek bir kırık hattı mevcut iken 12 olguda (%40) birden fazla alanda kırık vardı. Çocuklarda mandibulanın osteojenik potansiyeli yüksek olduğu için 16 olguda konservatif yaklaşım yeğlenirken geri kalan 14 olguda açık redüksiyon ve internal fiksasyon ile tedavi uygun görüldü. Olgular 4 ay ile 11 yıl arası takip edildi. Toplam 3 (%1) olguda komplikasyon tespit edildi ve hepsinde görülen komplikasyon arch barın gevşemesi idi.
Abstract
Mandibularfractures are rare in the pediatric population and carry different clinical features when compared with adults. To clarify the differences of etiology and patterns of fractures, a retrospective analysis of 30 pediatric mandibular fractures, treated between the years 1992-2003 in the Department of Plastic, Reconstructive and Aesthetic Surgery at Uludağ University Medical School, was presented. Cases were reviewed regarding the age, sex, etiologic factors, anatomic location of fractures and number of fracture sites, associated injuries, treatment methods and complications. Seven of the children were girls (23.3%) and 23 were boys (76.6%), and the mean age was 6.1 years. The reasons of mandibular fractures were falling from height in 13 cases (43.3%), motor vehicle accident in 12 cases (40%) and bicycle accidents in 5 cases (16.6%). In 18 cases (60%), we found only one site fracture and in 12 cases (40%) there was multiple site fractures. Sixteen cases (%53.3) sustained only mandibular fractures, 14 cases (%46.6) had associated injuries. The most common fracture sites wereparasymphisis (12 cases-40%) andcondyle (11 cases-36.6%). Because of the high osteogenic potential of pediatric mandible, we preferred to treat 16 cases by conservative methods and 14 cases by open reduction and internal fixation. The cases were followed up between 4 months and 11 years. Complications were encountered in only 3 cases (1%) and the complication was relaxation of the arch bar fixation.
