Özet

Göğüs duvarının tam katlı rezeksiyonlarını gerektiren tümörlerin büyük kısmını meme kanseri infiltrasyonları oluşturur. Thoraks duvarından kaynaklanan primer bağ dokusu tümörleri, akciğer kanserinin direkt yayılımı ve metastazları daha nadir nedenleri oluştururlar. Özellikle iskelet defektinin onarımı için geliştirilen sentetik materyaller ve lokal kas flebi imkanlarının ortaya konması göğüs cerrahlarına geniş rezeksiyon imkanını sağlamıştır. Çalışmaya 1997-2002 yılları arasında Şişli Etfal Eğitim ve Araştırma Hastanesi Plastik ve Rekonstrüktif Cerrahi Kliniği ile Siyami Ersek Hastanesi Göğüs Cerrahisi Kliniklerinin birlikte yaptıkları göğüs duvarının onkolojik, postenfeksiyoz rezeksiyonları ve rekonstrüksiyon uygulanan 7 olgu değerlendirilmiştir. Vakaların 6'sı göğüs duvarına ait tümörlerin rezeksiyonu, biri enfeksiyon drenajı için yapılan torakotomi sonrası oluşan defekt nedeniyle opere edilmişlerdir. 1 hastaya bilateral latissimus dorsi kas, 3 hastaya ipsilateral latissimus dorsi kas, 2 hastaya ipsilateral latissimus dorsi kas deri, 1 hastaya pektoral kas ve fasyakutanöz transpozisyon flebi kullanılmıştır. 1 hastada marjinal flep nekrozu ve 1 hastada minimal greft kaybı dışında yara iyileşmesi sorunu olmadı. Tüm hastalarda postoperatif dönemde solunum fonksiyonları yeterli olarak değerlendirildi. 1 hasta postoperatif 10. ayda nüks nedeniyle kaybedildi. Göğüs duvar rekonstrüksiyonu solunum fonksiyonunun ve derin hayati organların korunmasını birlikte amaçlayan komplike bir operasyondur.Bu ameliyatlarda kompleks rezeksiyon ve onarımların göğüs cerrahı ve plastik cerrahın işbirliği ile yapılması başarı şansını arttırır.

Abstract

Most of the cases demanding full thickness chest wall resection are the infiltrative carcinomas of the breast. Primary tumors of connective tissue, direct invasion of lung cancer and metastasis are presented rarely. Synthetic materials developed significantly for the reconstruction of skeletal defects and the use of local muscle flaps succesfully provides the thoracal surgeons resections to do extensive. We evaluated seven patients who went through resection and reconstruction of the chest wall due to malignancies or infections by the concomittant work of Department of Plastic and Reconstructive Surgery in Şişli Etfal State Teaching Hospital and Department of Thoracic Surgery in Siyami Ersek Hospital. Six of the patients operated for the defects due to tumoral resection of the chest wall while one due to defect following thoracotomy applied for infection drainage. Bilateral latissimus dorsi muscle flaps for one case, unilateral latissimus dorsi muscle flap for three cases, latissimus dorsi myocutaneus flap for two cases, pectoralis muscle flap andfasciocutaneus transposition flap for one case were the procedures applied. We had no problems about wound healing but only one marginal necrosis over the flap and one minimal elimination of graft over the flap at the postoperative period. None of the patients have respiratory disfunction after surgery. One case died as a result of recurrence at the 10th postoperative month. Reconstruction of the chest wall is a complex procedure aiming both preserving the respiratory functions and vital visceral organs. Either resection or reconstruction are the fields that plastic surgeons may take part.
