Distal saplı Peroneus brevis kas flebi ile bacak alt uç defektlerinin onarımı

Özet

Bacağın alt 1/3 yumuşak doku kayıpları büyük olduklarında sıklıkla serbest flepler gerektirirken küçük kayıpların kapatılması bölgesel fleplerle sağlanabilir. Peroneus brevis kası ideal yerleşimi itibarı ile distal fibula, anterolateral tibia ve aşil tendonu bölgesi onarımında kullanılabilir. Bacağın distal 1/3yerleşiminde doku defektleri bulunan 6 hastaya distal saplı peroneus brevis kas flebi ile rekonstruksiyon yapıldı. Peroneal arterden köken alan distal yerleşimli vaskuler dal görüldükten sonra kas flebi kaldırıp defektin olduğu bölgeye getirildi ve üzerine kısmi kalınlıktaki deri grefti uygulandı. Hastaların 4 'ünde herhangi bir problem yaşanmazken 1 'inde kısmi, 1 'inde ise tam flep kayıbı izlendi. Hastalar en kısa 1 ay en uzun 8 ay ortalama 5 ay takip edildiler. Hasta ve yara seçimi doğru yapıldığı taktirde distal saplı peroneus brevis kas flebinin bu bölge için uygun bir tercih olabileceğini düşünmekteyiz.

Abstract

The soft tissue defects, which are localized at the distal 1/3 of the cruris are reconstructed with free flaps when they are large in size, however, local flaps are used for smaller ones. Because of the ideal localization of the peroneus brevis muscle, it is preferred for reconstruction in the region of distal fibula, anterolateral tibia, and achilles tendon. The defects located in the distal 1/3 of the cruris were reconstructed with peroneus brevis muscle flap in six patients. After exploring the distally based vascular branch of the peronael artery, muscle flap was elevated and adapted to the defect, and a split thickness skin graft was applied on the muscle flap. Four of the patients didn t have any problem, while partial flap loss was seen in one patient. Total flap loss was observed in only one patient. We believe that peroneus brevis muscle flap may be an appropriate choice for the defects in the distal third of the leg if the selection of the patient and the defect is done accurately.
