Başparmağın farklı deformitelerinin onarımında 1. dorsal metakarpal arter ada flebinin kullanımı

Özet
Başparmak deformiteleri el fonksiyonlarını önemli derecede bozabilen ve düzeltilmesi oldukça güç olabilen problemlerden birisidir. Bu deformitelerin onarımında değişik lokal flepler kullanılabilmekle beraber, 1.dorsal metakarpal arter pediküllü ada flebi başparmağa ait farklı deformitelerin onarımında kullanılabilecek benzer ve yeterli yumuşak doku örtüsü sağlayan önemli bir lokal fleptir. Kliniğimize başvuran 21 hastada, başparmağı ve 1.parmak aralığını ilgilendiren çeşitli-deformitelerin düzeltilmesi amacıyla buflep kullanıldı. Dört hastada 1. parmak amputasyon güdüğünde geç onarım, 5 hastada 1. parmak aralığında yanık sonrası gelişen kontraktür açılması ve 12 hastada da I. parmakla gelişen deformitelerin düzeltilmesi buflep kullanılarak gerçekleştirildi. Operasyonlar aksiller anestezi ile ve turnike altında gerçekleştirildi. Tüm flepler duyulu ada flebi olarak kaldırıldı ve alıcı alana aktarıldı. Donör alanlar ise deri greftiyle onarıldı ve problemsiz iyileşti. Bir olguda parsiyel nekroz gelişti. Hastalar ortalama 1.5 yıl takip edildi ve sonuçlar kozmetik ve fonksiyel açıdan değerlendirildi. Başparmağın ve 1. parmak aralığının onarımlarında 1. dorsal metakarpal arter ada flebi, pratik ve güvenilir bir flep olamasın yanı sıra tatminkar fonksiyonel ve kozmetik sonuçlarıyla kullanışlı bir alternatiftir.
Abstract
Because of its well-known contribution to overall hand functions, posttraumatic deformities involving the thumb present one of the most challenging problems that impair the hand functions. Although many local flap alternatives have been described for correction of different deformities of the thumb, 1" dorsal metacarpal artery island flap is unique as the one that offers sufficient amount of similar tissue from a local source. In 21 patients, this versatile flap has been used for reconstruction of various deformities and defects involving the thumb and first web-space: for restoration of amputation stump in 4 patients, for release of post-burn contracture involving the first web space in 5 patients and for correction of various deformities in 12 patients. All surgeries were carried out with axillary anesthesia and under tourniquet. All of the flaps were harvested and transferred as sensate island flaps. Donor areas were covered with split thickness.skin grafts and healed without complication, while partial flap necrosis was experienced in one case. The mean follow-up period was 18 months and functional and cosmetic outcomes were evaluated at the end of the follow-up. In conclusion, use of 1st dorsal metacarpal artery flap in reconstruction of the defects and deformities involving the thumb and first web space is not only reliable and practical but also a versatile technique with satisfactory cosmetic and functional results.
