Sinir allogreft nakli ile sinir tamiri yapılan replante edilmiş kollar: İkinci yıllarının sonunda iki olgunun sunumu

Özet

Amaç: Sinir allogreft nakli ile sinir tamiri yapılan iki olgu, ikinci yıl sonunda sunulmaktadır. Olgular: Orta ve üst seviyede kol ampütasyonlu iki olguda (21 aylık ve 14 yaş) replantasyon yapıldı. Ulnar, median ve radial sinirler ağır ezilme ve çekilme hasarı nedeniyle sekonder onarıma bırakıldılar. Gereksinim duyulan sinir grefti miktarı dikkate alınarak her iki olguda da sinir allogreft nakli planlandı. İki olguda da proksimal sağlıklı güdükler infraklavikular yaklaşımla brakial pleksusun kordonları seviyesinde bulundu. Distal sağlıklı güdükler önkolda değişik seviyelerde bulundular. İlk olguda canlı vericilerden alınan sural sinir greftleri kullanıldı (Ocak 2004, 180 cm). Sonraki olguda kadavra greftleri kullanıldı (Mart 2004, 350 cm). Alıcı ve vericiler arasında sadece ABO kan grubu uyumu arandı. Alıcı ve vericilere organ nakli işlemleri aynen uygulandı. İmmünbaskılama, siklosporin, prednisolon ve azatiyoprin ile sağlandı. İlk hastada greftlerin distalinde yeterli sinir rejenerasyonu nedeniyle ilaçlar Ekim 2005 te kesildi. Sonuçlar: İlk olguda 18. ayda yapılan kaba gözlemlerde parmaklarda koruyucu duyu ve ekstrinsik fleksor reinervasyon görüldü. Ekstensorlar ve intrinsik kaslarda klinik reinervasyon bulgusu elde edilemedi. Diğer hastada Tinel testi bulguları elde ve parmaklarda değişik düzeylerde bulunmaktadır. Hiç motor inervasyon sağlanamayan hastada sadece koruyucu el duyusu elde etme beklentimiz vardır. Sonuç: Sinir noksanlığı otojen greftlerle onarılamayacak kadar fazla ise sinir allogreft nakli ilk seçenek tedavidir.

Abstract

SUMMARY Purpose: Two cases of nerve repair by nerve allograft transplantation are presented at the end of their second years. Cases: Replantation was performed in two cases with upper extremity amputations at middle and upper arm levels (ages 21 months and 14 years). Ulnar, median, and radial nerve disruptions were not repaired primarily because of severe avulsion. Nerve allograft transplantation was planned for both cases considering the total amount of nerve grafts needed. Healthy proximal ends were found at cord levels in infraclavicular exploration in both cases. Distal ends were at varying levels in the forearm. Sural nerve grafts harvested from living donors were used in the former patient (Jan 2004, 180 cm). Cadever nerves were transplanted in the latter case (Mar 2004, 350 cm). Only ABO blood group matching was required. Procedures followed in solid organ transplantation were completely implemented for both donors and recipients. Immunosuppression with cyclosporine, azothiopurine, and prednisolon was applied. In the former case, imminosuppression was halted in October 2005 because of apparently adequate regeneration across the nerve grafts. Results: Crude examination of the former case revealed protective sensation of the fingers and reinnervation of the extrinsic flexors, but no sign of force recovery in extensors and intrinsic muscles in 18 months. In the latter case, no motor recovery to any extent was obtained so far. Tinel tests are at varying levels in hand and fingers. We compromise for only protective sensation in hand for this case. Conclusion: Allogeneic nerve transplantation is the first choice, when the nerve defect is too extensive for autogenous nerve grafts.
