Özet

Deri greftlerinin sağ kalımı, birçok faktöre bağlıdır. Bunların başında greftin altında kan ve serum birikmesine bağlı olarak greftin alıcı yataktan havalanması ve yeniden damarlanmanın gerçekleşememesi gelir. Yeniden damarlanmanın başlarında greftin hareketsiz kılınması son derece önemlidir. Bazen geleneksel "tie-over" pansumanlar, düzensiz alıcı yataklar veya ulaşılması zor vücut bölgelerinde, greft yüzeyine dengeli bir bası uygulayamazlar ve yeterli hareketsizliği sağlayamazlar. Böyle durumlarda deri greftlerinin altında ölü boşluklar kalır ki buralar hematom veya seroma birikimi için riski arttırır. Bahsedilen nedenlerle deri kayıplarının önüne geçebilmek için birçok "tie-over" pansuman tekniği tanımlanmış olsa da hiç bir yöntem arzu edilen başarıyı sağlayamamıştır. Yazımızda "tie-over" pansuman olarak kullandığımız topikal negatif basınç tedavisi (TNBT) ile edindiğimiz deneyimler sunulmaktadır. TNBT deri greftleri üzerine dengeli bir bası uygular, greft altında sıvı birikimini azaltır ve yara salgılarını ortamdan uzaklaştırır. Sonuç olarak, klinik deneyimlerimiz TNBT'nin deri greftlerinin sağ kalım oranlarını arttıran bir yöntem olduğunu göstermektedir.

Abstract

Graft survival depends on numerous factors. The mostcommon cause of skin-graft failure is the collection ofblood or serous fluid under the graft, raising the graft fromthe wound bed and hence preventing revascularization.Strict immobilization of the graft during early period ofrevascularization is critical. Although a traditional tie-overdressing technique may be applied to support the take ofa skin graft. Sometimes, in difficult-to-reach locations andirregularly recipient sites, fixation is not adequate. In thissituation, dead spaces where hematomas and seromascould form are the main risk. Tie-over dressings and variousprotective devices to avoid skin graft disruption have beendescribed but none of them are perfect. In this paper, wepresent our experience with use of vacuum assisted closure(VAC) therapy as a tie-over bolster dressing for securingand maintaining skin grafts in good position. VAC therapyapplies uniform pressure over the entire surface of skin grafts,prevents fluid collection beneath the grafts and removes thewound fluids. In conclusions, our results suggest that theapplication of VAC therapy as a tie-over dressing improveoverall graft survival.
