Trigonosefali tedavisinde cerrahi teknik: 11 olgunun değerlendirilmesi

Özet
Metopik sütürün sinostozu sonucu oluşan trigonosefaliolgularının dış görünümünün normalleştirilmesi ve muhtemelkafa içi basınç artışının engellenmesi gibi estetik ve fonsiyonelnedenlerle ameliyat edilmesi gerekmektedir. Bu çalışmanınamacı trigonosefali olgularının tedavisinde kullandığımızfronto-orbital şekillendirme ameliyat yöntemini tanımlamakve tedavi sonuçlarını değerlendirmektir. Plastik Cerrahi veya Beyin Cerrahisi kliniklerine 1998-2006yılları arasında başvuran ve ameliyat edilen 11 trigonosefaliolgusunun (7-18 aylık, ort.= 13.2 ay) tümü bu çalışmayadahil edilmiştir. Ameliyatta frontal kemik ve supraorbitalsegment çıkarılmış, şekillendirilmiş, ilerletilmiş pozisyondatekrar yerleştirilmiş ve tel ve absorbe olmayan dikişmateryaliyle tespit edilmiştir.Tüm olgular ameliyat öncesive sonrası takiplerde (1-7 yıl, ort.=2.4 yıl) fizik ve nörolojikmuayene, fotoğraf, ve 3D-CT ile değerlendirilmiştir. Estetiksonuç değerlendirilirken hastada trigonosefali olduğunudüşündürecek hiçbir özellik kalmamış olması çok iyi, hafiftemporal depresyon dışında normal görünüm iyi,belirgintemporal depresyonun varlığı orta, orjinal deformiteninkısmen devamı veya belirgin asimetri kötü sonuç olarakdeğerlendirilmiştir. Olguların hiçbirinde erken postoperatifdönemde komplikasyon gelişmemiş, geç dönemde ise birolguda ekspoze olan tel çıkarılmıştır. Estetik sonuç 5 olgudaçok iyi, 5 olguda iyi ve bir olguda orta düzeyde bulunmuştur.Takip süresinde bütün olgularda nörolojik muayene bulgularınormal bir seyir izlemiştir. Hastaların büyük çoğunluğunda çokiyi ve iyi sonuçlar elde edilmiş olması ve takip sırasında önemlibir komplikasyon veya nüks görülmemiş olması uygulamaktaolduğumuz yöntemin etkinliğini ve güvenilirliğini ortayakoymuştur. Koronal insizyonun zigzag yapılışı, supraorbitalsegmentin tespitini kolaylaştıran üçgen çıkıntının oluşturulmasıgibi ayrıntılar olumlu sonuçlara katkıda bulunmuş, bunakarşın trigonosefalinin şiddetli olduğu bazı vakalardatemporal bölgedeki darlığın yeterince genişletilememesininkusursuz görünüm elde etmeyi engellediği gözlenmiştir.Trigonosefali olgularında kullanmakta olduğumuz yöntemgüvenilirliği ve estetik sonuçları açısından değerlendirilmiş,güvenilir ve estetik sonuçları bakımından etkili bulunmuştur.
Abstract
The patients with trigonocephaly need to be operated foraesthetic and functional reasons such as normalization oftheir apperience and prevention from increased intracranialpressure. The purpose of this study is to define the surgicalmethod that we use for reshaping of the fronto-orbital regionin patients with trigonocephaly and evaluate the results ofthis treatment.All of the11 patients with trigonocephaly (aged 7-18 months,mean=13.2) that were operated by the clinics ofplasticsurgery and neurosurgery between the years 1998 and2006 were included in the study. The surgery consistedof removal of the frontal bone and the supraorbital bar,reshaping them, placement of the reshaped bony partsat advanced position, and fixation using wires and non- absorbable suture material. All patients were evaluated withphysical and neurological examination,and photographswere taken both preoperatively and during the follow upperiod (1-7years, mean=2.4). A preoperative 3D-CTwas performed in every patient. The aesthetic result wasevaluated asvery good if no sign of the abnormality orof the operation other than the incisional scar was visible,as good if only a minor depression in the temporal regionswas visible, as fair if an obvious depression was seen in thetemporal regions, and bad if there was obvious asymmetryor any sign of the original deformity suggesting insufficientcorrection or relaps. There were not any complications inany case other than an exposed fixation wire in one patient,which needed removal. Aesthetic result was very good in 5patiens, good in 5 , and fair in one. All patients continued todo well both in physically and neurologically. The very goodand good results obtained in the majority of the patients,and the absence of any significant complications or anysigns of relapsing deformity observed during the follow upperiod demontrated that the surgical method that we usedwas effecient and safe. Some details in the surgery such asthe zigzag skin incision and the creation of a bony triangleat the end of the supraorbital bar, which fasciliated fixation,contrubuted to our good results. In some patients with severeanomalies however, insufficient widening of the narrowtemporal region prevented us from obtaining a perfectappearing result. The surgical technique that we used in thetreatment of patients with trigonocephaly was found to besafe and effective in obtaining good aesthetic results.
