Özet

Amaç: Birincil doğuştan hipotiroidi, sık görülen ve önlenebilir zihinsel gerilik nedeni olan bir endokrin hastalıktır. Çalışmamızda birincil do- ğuştan hipotiroidi olgularında, kalıcı/geçici birincil doğuştan hipotiroidi sıklığının saptanması, kalıcı birincil doğuştan hipotiroidi olgularında etiolojinin belirlenmesi, laboratuvar bulgularının tanıya yönelik katkısı- nın değerlendirilmesi amaçlandı. Gereç ve Yöntemler: Çalışmaya Ocak 2007- Haziran 2013 tarihleri arasında yenidoğan döneminde birincil doğuştan hipotiroidi tanısı alıp tiroksin tedavisi başlanmış ve izlenen üç yaşını doldurmuş 48 olgu alındı. Olguların tiroksin tedavisi altı hafta süresince kesildi ve bu sürenin sonunda tiroid hormonlarına bakıldı, tiroid ultrasonografisi yapıldı. Tiroid uyarıcı hormonu yüksek (>= 5 mIU/mL) bulunan olgulara tiroid sintigrafisi, sintigrafide iyot tutulumunun normal/artmış olması durumunda ise perklorat kovma testi yapıldı. Tiroid uyarıcı hormon düzeyi >=5 mIU/ mL olan olgular kalıcı birincil doğuştan hipotiroidi, altı ay boyunca normal olan olgular geçici birincil doğuştan hipotiroidi olarak tanımlandı. Bulgular: Çalışma anında olguların yaş ortalaması 3,8±0,7 yıldı. Tanı alma yaşı ortalaması 16,6±6,5 gün olup, 14 olgu (%29,2) Sağlık Bakanlı- ğı Tarama Programı ile tanı almıştı. Kalıcı birincil doğuştan hipotiroidi grubunda 23 olgunun (dokuzu erkek, 14'ü kız) 12'si (%52,2) disgenezi (sekiz hipoplazi, dört ektopi), 11'i (%47,8) dishormonogenezdi. Geçici birincil doğumsal hipotiroidi grubunda 25 olgu (17 erkek, 8 kız) vardı. Tanı anında grupların tiroid uyarıcı hormon düzeyleri benzerdi. Tedavi kesiminde kalıcı birincil doğuştan hipotiroidi grubunun almakta oldu- ğu tiroksin dozu geçici birincil doğuştan hipotiroidi grubundan daha yüksekti (sırasıyla;2,1±0,7, 1,5±0,5 mg/kg/g, p=0,004). Abstract

Aim: Primary congenital hypothyroidism is frequently seen endocrinedisorder and one of the preventable cause of mental retardation. Aimof study was to evaluate the frequency of permanent/transient hypothyrodism,and to detect underlying reason to identfy any marker whichcarries potential to discriminate permanent/transient form.Material and Methods: Forty eight cases older than 3 years of age, diagnosedas primary congenital hypothyroidism and started thyroxin therapyin newborn-period, and followed up between January 2007-June2013 were included in the study. Thyroid hormon levels were evaluatedand thyroid ultrasonography was performed in cases who are at the endof their 3 years of age, after 6 weeks of thyroxine free period. Thyroidsintigraphy was performed if serum thyroid-stimulating hormone washigh (≥ 5 mIU/mL) and perchlorate discharge test was performed if uptakewas normal or increased on sintigraphy. Cases with thyroid-stimulatinghormone levels ≥ 5 mIU/mL were defined as permanent primarycongenital hypothyroidism group and as transient primary congenitalhypothyroidism group with normal thyroid hormones during 6 months.Results: The mean age was 3.8±0.7 years. Mean diagnosis age was16.6±6.5 days and 14 cases (29.2%) were diagnosed by screening programof Ministry of Health. There were 23 cases (14F, 9M) in permanentprimary congenital hypothyroidism group and 12 (52.2%) of them weredysgenesis (8 hypoplasia, 4 ectopia), and 11 (47.8%) dyshormonogenesis.In transient primary congenital hypothyroidism group, there were 25cases (17M, 8F). The mean thyroid-stimulating hormone levels at diagnosiswere similar in two groups. The mean thyroxin dose in permanentprimary congenital hypothyroidism group was significantly higher thantransient group at the time of thyroxin cessation (2.1±0.7, 1.5±0.5 mg/kg/d, respectively, p=0.004). Thyroxin dose ≥1.6 mcg/kg/d was 72% sensitiveand 69.6% specific for predicting permenant primary congenitalhypothyroidism.Conclusions: Transient primary congenital hypothyroidism is morefrequent than expected and found often in males in the primary congenitalhypothyroidism cases, started thyroxin therapy in neonatal period.While fT4, thyroid-stimulating hormone, Tg levels at diagnosisdo not predict transient/permenant primary congenital hypothyroidism,thyroxin dose before the therapy cessation at the age of 3 maymake the distinction between transient/permenant primary congenitalhypothyroidism.
