Kliniğimizde Uygulanan Açık Parsiyel Horizontal Larenjektomilerin Avrupa Larengoloji Derneği’nin Yeni Sınıflamasına Göre Yeniden Değerlendirilmesi

Özet
Amaç: Bu çalışmanın amacı kurumumuzda gerçekleştirilen açık parsiyel horizontal larenjektomilerin (APHL), Avrupa Larengoloji Derneği'nin yeni sınıflaması göz önüne alınarak yeniden değerlendirilmesi ve yeni sınıflama sisteminin farklılıkları ile karşılaştı- rılmasıdır. Yöntemler: Kliniğimizde 2010 ve 2016 yılları arasında T1b, T2 ve T3 larenks kanseri tanısıyla APHL uygulanan 45 hastanın geriye dönük analizi yapıldı. Bulgular: Supraglottik larenjektomilerin (31 operasyon) hepsi Tip 1 APHL olarak sınıflandırılmakla beraber 11'inde ek yapılar rezeke edildi ve bunlar Tip 1 APHL + aritenoid (ARY) (5 operasyon), Tip 1 APHL + piriform sinüs (PIR) (4 operasyon), Tip 1 APHL + dil kökü (BOT) (1 operasyon), Tip 1 APHL + ARY + PIR (1 operasyon) şeklinde tanımlandı. Beş krikohiyoidoepiglottopeksili (CHEP) suprakrikoid larenjektomi, beş krikohyoidopeksili (CHP) suprakrikoid larenjektomi ve dört near total larenjektomi ameliyatı Tip 2 APHL (7 operasyon) ve Tip 3 APHL (7 operasyon) olarak yeniden sınıflandırıldı. Bu operasyonların üst insizyonu epiglotu içerdiği için, ikisi Tip 2b APHL ve dördü Tip 3b APHL olarak tanımlandı.Sonuç: Yeni sınıflama uygulanan ameliyatın başlığı- na, ameliyatın kapsamını da ekleyerek, farklı serileri ve teknikleri karşılaştırırken homojen bir terminoloji kullanılmasına imkan tanıyacaktır.
Abstract
Objective: The aim of this study was to re-evaluate the open partial horizontal laryngectomies (OPHLs) performed at our institution in terms of the new classification of the European Laryngological Society and compare the differences with the new classification system. Methods: A retrospective analysis of 45 patients diagnosed with T1b, T2, and T3 laryngeal carcinoma who were treated with OPHLs in our department between 2010 and 2016 were conducted. Results: All supraglottic laryngectomies (31 operations) were classified as OPHL Type 1, with 11 of them having additional structures resected and they were defined as Type 1 OPHL + arytenoid (ARY) (5 operations), Type 1 OPHL + piriform sinus (PIR) (4 operations), Type 1 OPHL + base of tongue (BOT) (1 operation), Type 1 OPHL + ARY + PIR (1 operation). Five supracricoid laryngectomies with cricohyoidoepiglottopexy (CHEP), five supracricoid laryngectomies with cricohyoidopexy (CHP), and four near-total laryngectomy operations were re-classified into Type 2 OPHL (7 operations) and Type 3 OPHL (7 operations). Among these operations, two were defined as Type 2b OPHL and four as Type 3b OPHL as superior incision included epiglottis. Conclusion: We consider that, this new classification allows for homogenous terminology when comparing different series and techniques by including the extent of the surgical removal in the operation title.
