Özet
Amaç: Bu çalışmada üçüncü basamak bir merkezde submandibuler bez eksizyonu yapılan olguların demografik bulgularının ve cerrahi sonuçlarının incelenmesi amaçlanmıştırYöntemler: Bu çalışmada 1997 ile 2014 yılları arasındasubmandibuler bez eksizyonu yapılan 45 olgununklinik özellikleri ve histopatolojik sonuçları ayrıntılıolarak değerlendirilmiştir.Bulgular: Hastaların 28'i (%62.2) ağrılı şişlik, 17'si(%37.8) ağrısız şişlik şikayeti ile başvurmuştur. Cerrahispesimenin histopatolojik incelenmesinde 14 hastada(%31.1) sialolitiazis, 16 hastada (%35.6) kroniksialadenit, 12 hastada (%26.7) benign tümörler, ikihastada (%4.4) malign tümörler, bir hastada (%2.2)ise ekstravazasyon mukoseli saptanmıştır. Komplikasyonlar, bir hastada (%2.2) fasial sinir marjinal mandibülerdalına ait kalıcı paralizi, yedi hastada (%15.6) fasialsinir marjinal mandibüler dalına ait geçici paralizibir hastada (%2.2) orokütanöz fistül, bir hastada da ise(%2.2) geçici hipoglossal sinir parezisidir.Sonuç: Bu çalışmada submandibuler bezde kitle şikayetiile başvuran hastalarda tanı çoğunlukla sialolitiazis,sialadenit ve benign kitleler olmuştur. Transservikalsubmandibuler bez eksizyonu, cerrahi tekniklereuyulduğunda ve seçilmiş hastalara uygulandığında,düşük komplikasyon ve nüks oranlarıyla yüz güldü-rücüdür..
Abstract
Objective: The aim of this study was to assess thedemographic findings and surgical results of patientswho underwent submandibular gland excision at atertiary care center.Methods: The clinical characteristics and histopathologicalresults of 45 patients who had undergone submandibulargland excision between 1997 and 2014were evaluated in detail.Results: Twenty-eight (62.2%) and 17 (37.8%) patientspresented with a complaint of a painful massand painless mass, respectively. Histopathologic investigationof the surgical specimen revealed sialolithiasisin 14 patients (31.1%), chronic sialadenitis in16 (35.6%), benign tumor in 12 (26.7%), malignanttumor in two (4.4%), and mucocele extravasation inone. As complications, permanent paralysis of the marginal mandibular branch of the facial nerve wasseen in one patient (2.2%), temporary paralysis of themarginal mandibular branch of the facial nerve wasseen in seven (15.6%), orocutaneous fistula was seenin one (2.2%), and temporary paralysis of the hypoglossalnerve was seen in one (2.2%).Conclusion: This study revealed that in patients presentingwith complaints of a submandibular glandmass, sialolithiasis, sialadenitis, and benign masseswere the mostly diagnosed pathologies. Transcervicalsubmandibular gland excision is a satisfactory procedurewith low complication and recurrence rateswhen it is performed on selected patients and obeyedto surgical techniques.
