Özet

(TKDG) stabilizasyonunda klasik bohça (tie-over)pansuman yerine boydan boya matris sütur uygulamasınıkarşılaştırmak ve greft başarısızlığıyla ilişkili riskfaktörlerini belirlemektir.Yöntemler: Bu tek merkezli geriye dönük çalışmada,on yıllık bir periyotta TKDG gerektiren baş ve boyunderi lezyonları eksizyonu yapılan hastalardan oluşanseri incelendi.Bulgular: Toplam olarak 128 TKDG rekonstrüksiyonuyapıldı. İzlem süresi 1 ile 192 hafta arasında değiş-ti. Greftin kabul oranı %86.4 olarak saptandı. Cerrahifiksasyon tekniği karşılaştırıldığında istatiksel olarakanlamlı bir fark bulunmadı. Yaş, cinsiyet veya defektalanının greftin kabul edilme oranında etkili faktörlerolmadığı tespit edildi. Sigara ve antikoagülan kullanı-mı ile greft başarısızlığı arasında bir ilişki bulunmadı.Sonuç: Geleneksel baskılı bohça pansuman tekniği ilekarşılaştırıldığında, greft dokusu ile ilgili çalışmayıve cerrahi süresini azaltarak, basit boydan boya matrissütür uygulaması yeterli greft kabulünü sağlar.

Abstract

Objective: To compare the outcomes of securingfull-thickness skin grafting (FTSG) with through-and-throughmattress suturing versus the classictie-over and pressure dressing and identify the associatedrisk factors of graft failure.Methods: A single-institution, retrospective case seriesof patients who had undergone excision of headand neck skin lesions requiring FTSG over a 10-yearperiod was reviewed.Results: In total, 128 FTSG reconstructions wereperformed. The follow-up period ranged from 1 to192 weeks. The observed graft take rate was 86.4%.There was no significant difference in the outcomewhen the surgical fixation technique was compared.Age, sex, or defect area did not affect the graft takerate. Smoking and the use of anticoagulants were notfound to be contributory factors to graft failure.Conclusion: Simple through-and-through mattresssuturing provides adequate graft take, while minimizingtissue handling of the graft and reducing surgicaltime in comparison to the traditional tie-over andpressure technique.
