Tıkayıcı Uyku Apne Sendromunun Tedavisinde Ekspansiyon Sfinkter Faringoplastinin Etkinliği

Özet
Amaç: Tıkayıcı uyku apne sendromu (TUAS) uyku sırasında tekrarlayan üst solunum yolu tıkanma atak- ları ve sıklıkla arteriyel oksijen satürasyonunda azalma ile karakterize bir sendromdur. Ekspansiyon sfink- ter faringoplasti (ESF) TUAS'ın cerrahi tedavisinde uygulanan basit, güvenli ve etkili bir yöntemdir. Ça- lışmamızın amacı TUAS nedeniyle ESF uygulanan hastalarda tedavi etkinliğinin polisomnografi (PSG) sonuçları ile değerlendirilmesidir. Yöntemler: Bu çalışma, 2010-2018 yılları arasında horlama, apne, tanıklı apne ve gündüz uykusuzluğu- nun tedavisi için merkezimize başvuran hastalar üze- rinde gerçekleştirildi. Öykü, fizik muayene ve PSG sonrasında cerrahi için uygun görülen ve cerrahiyi kabul ederek postoperatif 3. ayda PSG kaydı olan 67 hasta (16 kadın, 51 erkek) çalışmaya dahil edildi. Has- talar TUAS şiddetine göre hafif, orta ve ağır olmak üzere üç gruba ayrıldı. Hastaların yaş, cinsiyet, vücut kitle indeksi (VKİ), pre ve post-operatif dönem ap- ne-hipopne indeksi (AHİ), Epworth uykululuk ölçek skorları ve PSG verileri kaydedildi. Abstract
Objective: Obstructive sleep apnea syndrome (OSAS)is characterized by recurrent episodes of upper respiratory-tract obstruction during sleep and decreasein arterial oxygen saturation. Expansion sphincterpharyngoplasty (ESP) is a simple, safe, and effectivemethod for the surgical treatment of OSAS. The aimof our study was to evaluate the efficacy of ESP withpolysomnography (PSG) in OSAS patients.Methods: This study was conducted on patients referred to our center for the treatment of snoring, apnea,witnessed apnea, and daytime sleepiness during 2010-2018. Overall, 67 patients (16 females, 51 males) whohad PSG test at postoperative three months and wereconsidered suitable with history, physical examination, and surgery after PSG were included in the study.Patients were classified into three groups accordingto OSAS severity: mild, moderate, and severe.
