Adenoidektomide İntraoperatif Kanamayı Kontrol Etmede Hidrojen Peroksitin Hemostatik Etkisi

Özet
Amaç: Adenoidektomi güvenilir bir girişim olarak kabul edilmesine rağmen, kanama özellikle çocuklar- da en fazla görülen ve hayati tehlikesi olan bir kompli- kasyondur. Biz burada intraoperatif hidrojen peroksit irrigasyonunun pediatrik adenoidektomide operasyon süresi ve hemostaz üzerine klinik etkisini inceledik. Yöntemler: Bu çalışma adenoidektomi operasyonu geçiren pediatrik hastalarda hidrojen peroksitin he- mostaz üzerine etkisini inceleyen ileriye dönük, ran- domize, çift kör çalışmadır. Hasta, doktor ve çalışma hemşiresi hangi cerrahi teknik kullanıldığını bilme- mektedirler. Bulgular: Bu çalışmada ortalama yaş 5.46±1.19 (56 erkek ve 61 kız) olan ardışık 117 pediatrik hasta in- celendi. Hidrojen peroksit grubunda 58 hasta (or- tanca yaş 6 yıl, ortalama yaş 5.62±1.28 yıl) ve kontrol grubunda 59 hasta (ortanca yaş 5 yıl, ortalama yaş 5.31±1.07 yıl) vardı. İki grup arasında yaş (p=0.151), cinsiyet (p=0.646) ve adenoid büyüklüğü (p=0.7679 açısından belirgin farklılık yoktu. Abstract
Objective: Although adenoidectomy is generallyaccepted as a safe procedure, intraoperative hemorrhage is still the most common and potentially life-threating complication, especially in pediatricpatients. We evaluated the clinical effect of intraoperative hydrogen peroxide irrigation with respect tohemostasis and operation times in pediatric adenoidectomy.Methods: This was a prospective, randomized, double-blind study to investigate hydrogen peroxide solution in hemostasis in pediatric patients undergoingadenoidectomy. The patient, the surgeon, and thestudy nurse were blinded to the surgical techniqueused.Results: One hundred seventeen (56 males and 61females) consecutive pediatric patients with a meanage of 5.46±1.19 years were included in the study.There were 58 patients in the hydrogen peroxidegroup (median age: 6 years, mean age: 5.62±1.28 years) and 59 patients in the control group (medianage: 5 years, mean age: 5.31±1.07 years). No significant difference was observed between the two groupswith respect to age (p=0.151), gender (p=0.646), oradenoid size (p=0.767).
