Özet

Amaç: Bu makalenin amacı disfaji şikayetleri ile başvuran hastalarda tespit ettiğimiz, farinksin değişik seviyelerinde gözlenebilen çok nadir tümör ya da tümörbenzeri kitleleri tartışmaktır. Yöntemler: 2009-2011 yılları arasında hastanemize yut- ma güçlüğü şikayeti ile başvuran hastalar arasında disfajiye sebep olan orofaringeal, hipofaringeal ve laringealdüzeyde kitlesi bulunan 21 vaka teşhis edildi. Detaylı biranamnez ve fizik muayene ile birlikte tanı amaçlı olarakkontrastlı-kontrastsız Manyetik Rezonans Görüntüleme,Bilgisayarlı Tomografi, 3 boyutlu Bilgisayarlı Tomografi,Multiple row-Detector Bilgisayarlı Tomografi, MR an- jiyografi ve 99m Tc sintigrafi görüntülemeleri kullanıldı. Bulgular: On ikisi kadın ve 9u erkek 21 hasta calışmayadahil edildi. Yas ortalamasi 49.23±32.71 idi (min. 15-maks 80). On bir olguda, orafaringeal seviyede, kalan 10olguda ise laringeal ve hipofaringeal seviyede parsiyelobstrüksiyon saptandı.Sonuç: Endoskopik, radyolojik ve laboratuvar değerlendirmelerle birlikte detaylı hikaye ve muayene, disfajiyesebep olan orafarinks ve boynun az reastlanılan tümör vetümör benzeri lezyonlarının erken teşhisinde ve tedavi-sinde çok önemlidir.

Abstract

Objective: Te aim of this article is to discuss rarely en- countered tumours and tumorous masses at the diferentlevels of pharynx that were diagnosed in patients ad- mitted to our hospital with the complaint of dysphagia.Methods: Twenty-one cases with masses causing dys- phagia at the oropharyngeal, hypo-pharyngeal and la- ryngeal levels were identifed among patients admittedto our hospital with the complaint of difculty withswallowing between the years 2009 and 2011. De - tailed anamneses and physical examinations were per- formed. In addition, for the purpose of diagnosis, thefollowing were applied: Magnetic Resonance Imagingwith and without contrast, Computerized Tomography,Tree-Dimensional Computerized Tomography, Multi- ple row-Detector Computerized Tomography, MR An-giography and 99m Tc Scintigraphy.Results: Twenty-one patients, 12 women (57.1%) and9 men (42.9%), were included in the study. Teir meanage was 49.23±32.71 years old (min. 15-max. 80). Terewas partial obstruction at the oropharyngeal level in 11cases; there was partial obstruction at the laryngeal andhypopharyngeal level in the other 10 cases.Conclusion: A detailed history and examination alongwith endoscopic, radiological and laboratory evaluationsare important for early diagnosis and management ofrare tumours and tumour-like lesions of the oropharynxand neck causing dysphagia.
