Özet

Burun boşluğu ve paranazal sinüslerin granülomatözhastalıkları içerisinde enfeksiyöz nedenler önemli yertutmaktadır. Bakteriyel etmenlerden en sık mikobakte-rium türleri ve aktinomiçes, fungal patojenlerden en sıkaspergillus türleri ve daha nadiren mukor, kriptokok,histoplazma gibi ajanlar bu bölgedeki granülomatözenfeksiyonlardan sorumlu olmaktadır. Bu olgu sunu-munda 4 yıldır burun boşluğunda ve son aylarda dapremaksiller bölgede ciltte iyileşmeyen yarası olan, tanıve tedavide güçlük yaşanan hasta sunulmuştur. Birçokdefa biyopsi yapılmasına rağmen patolojik incelemey-le tanıya ve etkin bir tedaviye ulaşılamayan hastada,son cilt biyopsi materyalinden hem patolojik, hem de mikrobiyolojik değerlendirme yapılmış ve mantarkültüründe aspergillus üremesi sağlanmıştır. Böylecehastaya kronik nazal fungal enfeksiyon tanısı konarak,vorikonazol tedavisine başlanmıştır. Dokuzuncu ay-daki kontrolünde semptomların tama yakın düzeldiğigörülmüştür. Benzer vakalarda tanı da gecikilmemesiiçin patolojik örnekleme ile birlikte aerob, tuberkülözve mantar besiyerine uygun örnekler alınarak, granülo-matöz enfeksiyon etmenlerin araştırılması da ilk basa-makta akla getirilmelidir.

Abstract

Infectious diseases are important causes of granuloma-tous diseases of the nasal cavity and paranasal sinuses.Te most frequent bacterial agents responsible for gran-ulomatous infections are Mycobacterium species (spp.)and Actinomyces spp. In the case of fungal pathogens,Aspergillus spp. are the most frequent. Mucor spp., Cryp-tococcus spp. and Histoplasma spp. are the other fungalmicroorganisms causing granulomatous reactions. Inthis case report, a patient sufering from a chronic na-sal cavity lesion for 4 years with a recently developedpremaxillary skin lesion is presented. Despite many bi-opsies having been performed and pathological studiesconducted, a defnitive diagnosis and an efective treat- ment could not be achieved. Finally, Aspergillus growthwas observed in the fungal growth media when the ma-terial of the last skin biopsy was sent to microbiology aswell as pathology. Te patient was diagnosed as chronicnasal fungal infection and voricanozole treatment wasinitiated. Nine months after therapy, symptoms werealmost totally resolved. In order to prevent similar dif-culties of diferential diagnosis, biopsy materials shouldbe sent for aerobic, tuberculosis and fungal cultivation,as well as pathological examination, in order not to missinfectious diseases from the aetiology.
