Pediatric tracheotomy: A relatively rare indication limited to pediatric intensive care subjects?

Özet
Amaç: Pediatrik trakeotomi uygulanmış olgularda endi- kasyonların, altta yatan sebeplerin ve trakeotomi sonrasısonuçların değerlendirilmesi. Yöntemler: Ocak 2004 ve Eylül 2013 tarihleri arasındapediatrik trakeotomi uygulanmış olgular çalışmaya dahiledildi. Tüm olgular 14 yaşın altındaydı. Olguların bi- rincil hastalıkları, trakeotomi endikasyonu, yaş, cinsiyet,komplikasyon tipi kaydedildi.Bulgular: Pediatrik trakeotomi uygulanmış kırkbeş olgubelirlendi. Olguların yaş ortalaması 26,3 ay idi (2-140 ay).Yirmibir olgu (n=21-%46,6) bir yaş altında idi. Otuz dörtolguda (n=34-%75,5) trakeotomi endikasyonu uzamışentübasyon idi. Kalan onbir (n=11-%24,4) olguda trake- otomi endikasyonu üst solunum yolu obstrüksiyonuidi. Kırk beş olgunun onüçünde (n=13-%28,8) erken kompikasyonlar, kırk beş olgunun ikisinde (n=2-%4,5)geç komplikasyonlar izlendi. Erken dönemde bir olgutrakeotomi ile ilişkili sebeplerden kaybedildi. Çalışmasüresince sekiz olgu (%18,1) altta yatan hastalıklar ned-eniyle kaybedildi. Onüç olgu (%36,1) başarılı bir şekildedekanüle edildi. Kalan 23 olgu (%63,8) dekanüle edi-lemedi.Sonuç: Bu çalışmada pediatrik trakeotomi uygula-malarının tümü çocuk yoğun bakım ünitesinde elek-tif olarak yapıldı. Hiçbir trakeotomi akut üst solunumyolu tıkanıklığı için yapılmadı. Pediatrik trakeotomininplanlı ve elektif olarakta yapılsa hala önemli morbiditeve mortaliteye sahip olduğu görüldü.
Abstract
Objective: To evaluate indications, underlying condi-tions and outcome in pediatric tracheotomy subjects.Methods: Between January 2004 and September 2013,pediatric subjects who underwent a tracheotomy oper-ation were included for study. All subjects were underthe age of 14. Subjects primary diseases, indication oftracheotomy, age, gender and type of complications wererecorded.Results: Forty fve subjects were identifed. Te meanage of subjects was 26.3 months (range between 2 to140 months). Twenty one (46.6%) subjects were underthe age of 1 year. In 34 (75.5%) out of 45 subjects, tra-cheotomy indication was prolonged intubation. In theremaining 11 (24.4%) subjects, tracheotomy indicationwas upper respiratory tract obstruction. Tirteen (n=13-28.8%) out of 45 experienced early complications and 2 (n=2-4.5%) out of 45 subjects experienced late compli-cations. One tracheotomy related death occurred in theimmediate period. Eight (18.1%) of subjects died duringthe study period from underlying conditions. Decannu-lation was successfully performed in 13 (36.1%) subjects.In the remaining 23 (63.8%) subjects, decannulationcould not be done.Conclusion: All tracheotomies in this report were per-formed on pediatric intensive care subjects on an elec-tive basis. No tracheotomy was performed for acute up-per airway obstruction. Even when planned and electivetracheotomy is performed it still has signifcant mortal-ity and morbidity.
