Anatomic and functional results of cartilage type 1 tympanoplasty in children and the evaluation of the success of surgery with parental perceptions

Özet
Amaç: Çocuk hastalarda kartilaj Tip 1 timpanoplastiameliyatının anatomik ve fonksiyonel sonuçlarını belir-lemek ve timpanoplasti ameliyatının başarısını ebeveynalgısı ile değerlendirmek.Yöntemler: Çalışmaya, kartilaj Tip 1 timpanoplastiameliyatı yapılan çocuk yaş grubu hastalar dahil edildi.On altı yaşından büyük ve son 1 ayda kulak akıntısı olanhastalar, kolesteatomalı kronik otitler, mastoidektomive ossiküloplasti yapılan hastalar çalışma dışı bırakıldı.Postoperatif en erken 6. ayda yapılan son kontrolde, in-takt greft anatomik başarı olarak değerlendirildi. Has-taların ebeveynlerine telefonla ulaşılarak, yapılan cerrahiişlemin sonuçları ile ilgili algılarına yönelik kendilerinedört soruluk bir memnuniyet anketi uygulandı.Bulgular: Çalışmaya 22 erkek 19 kız çocuk 41 hastanın 44kulağı dahil edildi. Hastaların yaş ortalaması 13.2±2.7 (8- 16) ortalama takip süresi ise 17 (7-36) ay idi. Anatomikbaşarı oranı %88.6 (39/44) olarak tespit edildi. Pre-operatifsaf ses işitme eşiği ortalaması 30.7±7.7 dB iken, post-ope-ratif 18.8±7.8 dB olarak tespit edildi. 41 çocuk hastadan28nin (%68.2) ailesine telefonla ulaşılabildi. 28 hastanın19u (%68), ameliyat sonrası işitmesinde artma olduğunudüşünürken, sadece 10u (%35) ameliyat sonrası daha önceyapamadığı aktivitelere başladığını belirtti.Sonuç: Çocuk hastalarda kartilaj timpanoplasti sonuçlarıhem cerrahı hem de ebeveynleri tatmin edicidir ve opti-mum koşullar geliştiğinde, mümkün olan en kısa süredeoperasyon planlanması uygundur.
Abstract
Objective: Tis study aims to determine the anatomicand functional results of cartilage Type 1 tympanoplastyin children and to evaluate the success of surgery withparental perceptions.Methods: Only pediatric patients who had undergonecartilage Type 1 tympanoplasty were included in thisstudy. Patients older than 16 years and patients who hadotorrhoea during the previous month, chronic otitis me-dia with cholesteatoma and patients who had undergonemastoidectomy or ossiculopasty were excluded. Te an-atomical success was accepted as an intact graft in thelast control in the at least 6 th month postoperatively. Asatisfaction questionnaire with four questions about theperceptual results of surgical intervention was applied tothe patients parents on the telephone.Results: In total, 44 ears of 41 cases (22 male 19 fe-male) were included in the study. Te mean age was13.2±2.7 (8-16) and the mean follow-up time was 17 (7-36) months. Anatomical success rate was 88.6%(39/44). As a preoperative mean, the pure tone hearingthreshold was 30.7±7.7 dB and the postoperative meanpure-tone hearing threshold was 18.8±7.8 dB. Amongthe operated ears, 88.6% (39/44) of the perforationswere treated successfully. Of the 41 paediatric patients parents, only 28 (68.2%) could be reached by telephone;nineteen (68%) of the 28 parents contacted thought thatthe hearing level had improved postoperatively and only10 parents (35%) stated that his/her child could attend apreoperatively-restricted activity postoperatively.Conclusion: Outcomes of cartilage tympanoplasty inchildren satisfy both surgeons and parents. Chronic oti-tis media in children must be managed with surgery assoon as favourable conditions are provided.
