Why do we not need ct in patients who undergo type i tympanoplasty?

Özet

Amaç: Yüksek çözünürlüklü bilgisayarlı tomografi (YÇBT) süpüratif kronik otitlerin tanı aşamasındayaygın kabul görmektedir. Çoğu kulak cerrahı, özellikledirençli ve kötü kokulu kulak akıntısı, retraksiyon ceplerinde epitelyal debris ve orta kulağın kemik duvarlarında defekt olan hastalarında doğru bir ameliyat planıoluşturmak için rutin YÇBT isteğinde bulunmakta-dırlar. Bu çalışmanın amacı Tip 1 Timpanoplasti ya-pılan hastalarda, ameliyat öncesinde YÇBT istemeningereksiz olduğunu prediktif değeriyle birlikte ortayakoymaktır.Yöntemler: Bu çalışma normal orta kulak mukozası ilebirlikte olan kuru perforasyon nedeniyle timpanoplastiuygulanan bir hasta grubunu içermektedir. Toplam 43hastanın intraoperatif bulguları, ameliyat öncesi YÇBTve odyogram bulguları ile karşılaştırılmıştır.Bulgular: Ameliyat öncesi hava yolu eşikleri ile YÇ- BTde kemikçik erozyonu görülmesi arasında anlamlıilişki yoktur (p=0,083). YÇBT, iki hastada kolesteatommevcudiyetini hatalı tahmin etmiştir. YÇBTnin yumuşak doku veya granülasyon dokusunu tespit etmedekipozitif prediktif değeri 4/14 (%28,57) olmuştur. YÇ-BTnin kemikçik zincirin durumu için tanısal duyarlılığı%9,09 ve özgüllüğü ise %90,62 olarak bulunmuştur.Sonuç: YÇBT, kolesteatomu ve süpüratif kronik ortakulak enfeksiyonu olan hastaların değerlendirilmesindeönemli bir rol oynamaktadır. Ancak tip 1 timpanoplastiyapılan hastalarda ameliyat öncesinde YÇBT istenilmesi gereklideğildir. Bu durumlarda faydası olmadığı gibihastaların gereksiz radyasyona maruz kalmasına sebepolmaktadır.

Abstract

Objective: High-resolution computed tomographyHRCT) has gained wide acceptance in diagnosticwork-up of suppurative chronic otitis media. Most earurgeons order HRCT to make a rational operative plan,specially in the presence of symptoms, such as refrac- ory otorrhea with bad odor, epithelial debris at retrac- on pockets, and defects in bony walls of the middle ear.Te aim of this study was to show the lack of necessity ofreoperative HRCT by evaluating its predictive value inatients who underwent type I tympanoplasty. Methods: Te study included a patient group who un- erwent tympanoplasty with dry perforation and nor- mal middle ear mucosa. Te intraoperative fndings of3 patients were compared with preoperative HRCTnd audiogram fndings. Results: Tere was no signifcance between preoperativeir conduction threshold and existence of ossicular ero- sion in HRCT (p=0.083). HRCT incorrectly predictedthe presence of cholesteatoma in two patients. Te posi-tive predictive value of HRCT in detecting soft or gran-ulation tissue was 4/14 (28.57%). Te sensitivity andspecifcity of HRCT in the diagnosis of ossicular chainstatus were 9.09% and 90.62%, respectively.Conclusion: HRCT plays an important role in the as-sessment of patients with suppurative chronic otitis me-dia and cholesteatoma. However, preoperative HRCTin patients who have undergone type I tympanoplasty isnot necessary. It has no beneft and also leads patients tounnecessary radiation exposure.
