Stapes surgery outcomes: Te practice of 35 years

Özet

Amaç: Bu çalışmanın amacı Hacettepe ÜniversitesiKulak Burun Boğaz ve Baş Boyun Cerrahisi Anabilim- dalında stapes cerrahisi yapılan hastaları cerrahi tipi,kullanılan protez, revizyon oranı ve odyolojik değerlen- dirmeler ışığında özetlemektir. Yöntemler: Otuz beş yıllık data taranmış ve stapes cer- rahisi yapılan hastalardan 327sinin verilerine ulaşılmış- tır (190 kadın, 137 erkek). Yaş aralığı 11-70 arası değiş- mekteydi (ortalama: 39). Kliniğimizde stapes cerrahisi çoğunlukla transmeatal in- sizyonla yapılmakta, orta kulağa girildikten sonra hemenher zaman stapes mobilitesi değerlendirilmektedir. Mal- leus ve inkus hareketli ancak stapes fikse olduğu vakalar- da; küçük bir perforasyon şeklinde stapedotomi yapılarak,inkus uzun kolu ve stapedotomi arasına protez yerleştiril- mektedir. Stapedotomi kemik talaş ile desteklenmektedir.Preoperatif ve postoperatif odyogramlarına ulaşılabilen199 hastanın 500, 1000, 2000 ve 4000 Hz hava-kemikiletimleri ortalaması alınarak karşılaştırılmıştır. Bulgular: Stapes cerrahisi yapılan 327 hastanın %62sin- de hava-kemik iletim farkı 10 dBin altında, %88inde 15dBin altında ve %94ünde is 20 dBin altında bulun-muştur. Hastaların %6sında 20 dBden fazla hava-kemikyolu aralığı olduğu saptanmıştır. Yirmi hastada revizyoncerrahi gerekliliği olmuş ve bu olguların hava kemik iletifarkı %60ında kapatılmıştır. Bir hastada ani sensörinö-ral işitme kaybı ve şiddetli vertigosu olan 1 hastada daperilenf fistülü gelişmiştir.Stapedotomi ve kemik talaş ile tefon piston kullanımı-nın otoskleroz cerrahisinde ve iletim tipi işitme kaybınındüzeltilmesinde oldukça başarılı olduğu düşünülmektedir.İleri sensörinöral işitme kaybı olan ve oldukça az ve yaölçülemeyen hava kemik iletim fark olan olgularda da bucerrahinin kontraendikasyon oluşturmadığı saptanmıştır.Sonuç: Stapes cerrahisi (stapedektomi ve ya stapedotomi)otoskleroz kaynaklı iletim tipi işitme kayıplarının düzel-tilmesinde başarılı bir yöntemdir. Değişen oranlarda totalsensorinöral işitme kaybı, fasial sinir paralizi ve perilenf fis-tülü gibi komplikasyonlar açısından cerrah dikkatli olmalı,hastalar cerrahi öncesi bu açıdan da bilgilendirilmelidir.

Abstract

Objective: Te objective of this study was to review pa-tients who underwent stapes surgery in the HacettepeUniversity Ear Nose Troat and Head and Neck Sur-gery Department with subgroups, such as type of sur-gery, prosthesis used in the surgery, rate of revision, andaudiological results.Methods: Te data of 35 years were searched, and it wasdetected that 327 patients (190 females, 137 males) hadundergone stapes surgery; the age was ranging between11-70 years (mean: 39).In our clinic, stapes surgery is performed mostly by trans-meatal incision. After entering the middle ear, we almostalways look for stapes mobility. If there is mobility in theincus and malleus and no mobility in the stapes, a smallfenestra is performed as stapedotomy and prosthesis andplaced between the stapedotomy fenestra and incus longarm. To cover the space near the stapedotomy, small bonyfragments are placed. In patients who had preoperativeand postoperative audiograms, the mean value of 500,1000, 2000, and 4000 Hz air-bone conduction thresholdsof 199 cases were measured and compared.Results: In 62% of 327 patients who were operated on,the air-bone gap was less than 10 dB, less than 15 dB in 88%, and less than 20 dB in 94%. In 6% of patients,there was air-bone conduction gap of more than 20 dB.Revision surgery was performed in 20 patients. Of them,air-bone gap closure was achieved in 60%. One patienthad sudden sensorineural hearing loss. Tere was also aperilymph fstula in one patient who had vertigo.Stapedotomy and using a Tefon piston with autogenousbone are successful methods of recovering conductive-type hearing loss in otosclerosis. In patients with ad-vanced sensorineural hearing loss, a very low air-bone gapor unmeasured air-bone gap is not a contraindication.Conclusion: Stapes surgery (stapedectomy or stapedot-omy) is a successful surgery in the case of conductive-typehearing loss with otosclerosis. Complications, such as totalsensorineural hearing loss, facial nerve paralysis, and peri-lymph fstula, could be seen at variable rates, and the sur-geon should be cautious, and the patients must be informedabout these complications in the pre-operative period.
