Özet

Osteoid osteoma en sık 7-25 yaş grubunda rastlanan benign bir tümördür. Genellikle erkeklerde görülen, en sık alt ekstremitelerin uzun kemiklerini tutan, değişik klinik semptom ve bulgularla seyredebilen osteoid osteomanın en sık bulgusu gece artan ağrıdır. Ağrı paterni bu hastalıkta zaman zaman radiküler ağrı ile karışabilmekte ve tanıda güçlük yaratabilmektedir. Bu makalede proksimal femurde osteoid osteoma saptanan 13 yaşında bir erkek hastanın sunulması ve tartışılması amaçlanmıştır. Hasta kalça ve uyluk ağrısı ve uyluk ön yüzünde uyuşma yakınmaları ile bir çok merkeze başvurmuş ve değişik tanılarla 5 ay süreyle takibe alınmıştır. Hastadan çekilen kalça manyetik rezonans görüntüleme bulguları stres kırığı olarak yorumlanmış, ancak sonrasında yakınmalarının giderek artması, medikal tedaviye ve istirahate yanıt vermemesi nedeniyle bölümümüze baş vuran hastanın muayenesi sonucunda çekilen bilgisayarlı tomograde osteoid osteoma tanısı konulmuş ve perkütanöz radyofrekans ablasyon tedavisi planlanmıştır. Osteoid osteoma değişik klinik tablolar ile seyredebildiği için ağrı ile gelen genç hastalarda ayrıcı tanıda göz önünde bulundurulması yararlı olacaktır.

Abstract

Osteoid osteoma is an osteoblastic benign tumor of the bone which is generally seen between the ages of 7-25 years. It is usually seen in males, especially in long bones of the lower extremities, with various clinical symptoms and signs. The most common symptom is severe pain that is often observed at night. The pattern of pain may easily be confused by radicular pain and thus making the diagnosis difcult. In this report, we present a 13-year-old boy with osteoid osteoma involving the lesser tubercle of femur. The patient complained of hip and thigh pain and numbness in the anterior of the thigh. He had been previously seen in several hospitals with various diagnoses in the last 5 months. On hip magnetic resonance imaging, stress fractures were identied. Since the symptoms were not relieved with rest and medical treatment, he was referred to our clinic and computed tomography conrmed the diagnosis of osteoid osteoma. Finally, the patient was planned to be treated with percutaneous radiofrequency ablation. We aimed to emphasize that osteoid osteoma might have different clinical symptoms and that we should consider it especially in young patients with relevant painful complaints.
