Özet
Bu olgu ile amacımız etiyolojik faktörleri arasında doğrudan yer almayan yüksek doz kortikosteriod kullanımının da tek başına sekonderprotrüzyo asetabuli yapabildiğini göstermektir. Protrüzyo, asetabuli pelvis anteroposterior grafde asetabular çizginin, ilioiskial çizgiyi erkeklerde3 mm, kadınlarda ise 6 mm geçmesi ile belirlenir. Etiolojiye yönelik birçok araştırma yapılmış ancak tek ortak faktör bulunamamıştır. 1939daGilmour tarafından altta yatan bir patoloji gösterilemeyenler primer grup, altta yatan herhangi bir patolojiye bağlı gelişenler sekonder grupolarak sınıfandırılmıştır. Yetmiş üç yaşında kadın hasta, iki yıldır sağ kalçada ağrı, kısıtlılık ve yürüme güçlüğü şikayetleri vardı. Dört yıl önceeozinoflik pnömoni tanısıyla bir buçuk yıl (1 mg/kg/gün) prednizolon tedavisi almıştı. Devamlı olarak ise salmeterol+futikazon (500/50 mcg)2x1 kullanıyordu. Kemik mineral yoğunluğu ve diğer laboratuvar bulguları normal idi. Hastanın sağ kalça ağrısının nedeni, steroid kullanımınabağlı iki yıl içinde hızla progrese olan protrüzyo asetabulive buna sekonder gelişen Osteoartrit olarak değerlendirildi. Bildiğimiz kadarıyla buolgu literatürde ilktir. Bu tür hastaların kliniklerinin kısa sürede progresyon gösterebileceğini de unutmamak gerekir. Bu nedenle kalça ağrısı ilegelen hastaların hikayeleri alınırken kullanmış oldukları ilaçlar daha detaylı sorgulanmalı ve tedavilerinin de ona göre belirlenmesi gerektiğinisöyleyebiliriz. Abstract
The aim of presenting this case was to demonstrate that using high doses of corticosteroids which are not directly involved in etiologicalfactors can cause secondary protrusio acetabuli. Protrusio acetabuli is identifed with an acetabular line projecting ilioischial line for 3 mmin males and 6 mm in females on antero-posterior radiographs of the pelvis. This is the frst case seen in the literature, despite variousstudies conducted about the etiology, no common factor was found. In 1939, Gilmour classifed them as following; the group, in whichany underlying pathology can not be shown as primary, and the group that progress protrusio as a result of any underlying pathology assecondary. The patient was a 73 year-old woman with pain, loss of ROM in right hip and diffculty in walking for 2 years. Four years ago witha diagnosis of eosinophilic pneumonia she received prednisolone (1 mg/kg/day). She was constantly using salmeterol+futicasone (500/50mcg) ) twice daily. Bone mineral density and other laboratory fndings were normal. The cause of the patient s right hip pain was associatedto the acetabular protrusion which progressed rapidly within two years due to use of steroid and osteoarthritis progressing secondarily toacetabular protrusion. To the best of our knowledge this case is seen in the literature for the frst time. It should not be disregarded that suchpatients may show clinical progression in a short time. Therefore, drug history of the patients with hip pain should be evaluated in detail andtreatments should be determined accordingly. (Turkish Journal of Osteoporosis 2015;21: 34-6)
