Distraksiyon osteogenesis yöntemiyle premaksillanın ileri alınmasının ve TGFB1'in kemik yapımı üzerine etkilerinin incelenmesi

Özet

Amaç: Bu deneysel çalışmanın amacı, kemik-dis destekli distraktoı kullanımı ile premaksillanın ileri alınmasının kata yöz yapıları üzerine etkileri ile TGFBl'in kemik iyileşmesi üzerine olan etkilerinin incelenmesidir. Gereçler ve Yöntem: On-dörr adet tavşan 3 gruba ayrılarak 1 .grup sadece distraksiyon ostogenesis uygulanan 7 tavşandan, 2.grup distraksiyon os-tûgenesis+ TGFftl uygulanan 5 tavşandan, kontrol grubu ise 2 tavşandan oluşturuldu. Anestezi ve distraktoı ün uyum-lanmasını takiben palatal kemikte ve maksillanın lateral duvarında kortikotomi yapıldı. Cerrahiden 3 gün sonra, distrak-tör vidası 8 gün boyunca günde bir kere, 0,6 mm aktivasyon olacak şekilde açıldı. İkinci gruptaki tavşanlarda vida açılmaya başlandıktan 2-1 saat sonra kortikotomi yapılan bölgeye lokal olarak TGFftl uygulandı. Tavşanlar 12 gün sonra sakrit'iye edilerek histolojik değerlendirme için örnekler alındı. Bulgular: Distraksiyon os-togenesis uygulanan grupta 3,79 mm; distraksiyon ostogenesis ile birlikte TGF81 uygulanan grupta 3.70 mm, premaksillanın ileri doğru hareket ettirildiği saptandı. Sonuç: Her iki grupta sefalo-metrik bulgular benzer bulunurken TGFfsl uygulanan grupta histolojik olarak kemik iyileşmesinin daha iyi olduğu gözlendi.

Abstract

Aim: The aim ot this experimental study was to evaluate the advancement of premaxilla with a bone-tooth borne distractors using the landmarks on standard lateral cephalograms and the effects or TGF81 on bone healing. Materials and Methods: Fourteen rabbits were divided in 3 groups where first group (n=7) was treated with distraction osteogenesis, second group (n=5) was treated with distraction osteogenesis combined with external TGF&I application, and 2 rabbits were treated only with corticotomy a s c ontrols, following anesthesia and adaptation of the individual distractors, corticotomy of palatal bone and lateral maxil-lary walls for distraction was made. The distraction has started 3 days following the surgical procedure, and the device was activated 0.6 mm each day for 8 days. External TGFfll application was made following 24 hours of the initial expansion in animals where distraction was combined with the application of growth factor. Animals were sacrificed after J2 days of retention period and specimens were collected for histological examination. Results: The amount of anterior movement of the premaxilla was found 3.79 mm for the distraction osteogenesis group and 3.7 mm tor the group where external TGF01 application was made following distraction. ConcOeft palate, alveolar distraction, secondary alveolar bone grafting.ups, histological bone healing was much better in TGFRI treated group.
