Şaşılık operasyonlarında ayarlanabilir sütür tekniğinin etkinliği

Özet
Amaç: Kliniğimizde ayarlanabilir sütür tekniği ile öpere edilen hastalarda, operasyon sonuçlarının araştırılması. Gereç ve Yöntem: İstanbul Beyoğlu Göz Eğitim ve Araştırma Hastanesi çocuk gözü ve şaşılık biriminde 11.1.1995 ile 27.1.2003 tarihleri arasında öpere edilen hastaların dosyalan retrospektif olarak incelendi. Bulgular: 54 nonparalitik şaşılık hastası ayarlanabilir sütür tekniği ile öpere edildi. Hastaların preoperatif tanıları 20'sinde esotropya (ET), 34'ünde ekzotropya (XT) idi. ET olgularının 17'sine primer cerrahi, 3'üne reoperasyon, XT olgularının 25'ine primer cerrahi, 9'una reoperasyon olarak ayarlanabilir sütür cerrahisi uygulandı. Operasyonlarda ayarlanabilir sütür tekniği ile 20 iç rektusa ve 34 dış rektusa, 49 geriletme, 4 rezeksiyon ve 1 avansman uygulandı. Cerrahiden 4-24 saat sonra yapılan muayene sonucunda, 45 hastada sütür aynen bağlandı, 9 hastada ayarlama gerekti. Tüm şaşılıklarda %72.97, ET olgularında %71.03, XT olgularında %74.74 oranında düzelme saptandı. ET ve XT grupları, primer ve reoperasyon alt grupları arasında düzelme açısından istatistiksel farklılıklar anlamsızdı (p=0.285, p=0.618, p=0.591). %8.3 hastada yeniden cerrahi düzeltme gerekti. Sonuç: Ayarlanabilir sütür tekniği uygulanan vakalarda tek bir operasyon ile tatminkar sonuç alınabilen, etkili bir teknik olduğu kanaatine varılmıştır.
Abstract
Purpose : To investigate the surgical results in patients operated with adjustable suture technique in our clinics. Methods : Records of the patients who where operated between 11.1.1995 and 27.1.2003, in Istanbul Beyoglu Eye Education and Research Hospital pediatric eye and strabismus department, were analyzed retrospectively. Results : 54 nonparalytic strabismus patients were operated with adjustable suture technique. Preoperative diagnosis of patients were esotropia (ET) in 20, exotropia (XT) in 34 cases. Adjustable suture surgery was performed in the ET cases as primary surgery in 17 and as reoperation in 3 patients; whereas in the XT cases as primary surgery in 25 and as reoperation in 9 patients. In the operations, 49 recession, 4 resection and 1 advancement were applied with the adjustable suture technique, on 20 medial recti and 34 lateral recti. According to the result of the examination performed 4-24 hours after the surgery, sutures of 45 patients were tied in the same position, while 9 patients required adjustment. Correction was determined in 72.97%, of the overall strabismus, in 71.03% of the ET cases and 74.74% of XT cases. Among the ET and XT groups, primary and reoperation subgroups, there were no statistically significant difference with respect to correction (p=0.285, p=0.618, p=0.591). In 8.3% of the patients, surgical correction repetition was required. Conclusion : Adjustable suture technique is determined to be an efficient technique, with which satisfactory results can be obtained with only one operation.
