Graves oftalmopatide uygulanan orbita dekompresyonunda güncel cerrahi teknik ve endikasyonlar: Sekiz yıllık deneyimlerimiz

Özet

Amaç: Graves oftalmopatili olgularda kozmetik ve fonksiyonel nedenlerle uygulanan orbita dekompresyonu cerrahisinde elde edilen gelişmeleri ve bu yaklaşımlar ile sağlanan sonuçları vurgulamaktır. Yöntem: 1994-2001 tarihleri arasında orbita dekompresyonu cerrahisi uygulanan 35 olgu (56 göz) retrospektif olarak değerlendirildi. Beş olgunun 9 gözüne iç ve alt duvar, 7 olgunun 13 gözüne iç, alt ve dış duvar, 13 olgunun 18 gözüne dengeli iç ve dış duvar dekompresyonu ve 10 olgunun 16 gözüne iç ve dış duvarda uygulanan dengeli kemik dekompresyonuna ek olarak orbita yağ dokusu eksizyonu uygulandı. Alt duvar alınarak ve alınmadan dekompresyon yapılan olgularda, postoperatif yeni diplopi gelişme oranları istatistiksel olarak student t test ile karşılaştırıldı. Bulgular: Orbita dekompresyonu uygulanan olguların 22'si kadın, 13'ü ise erkek olup yaşları 20-69 yıl (ortalama 42.9 yıl) arasında bulundu. Olgular 3-72 ay (ortalama 36.8 ay) izlendi. Hiçbir olgu takipten çıkmadı. Proptozis düzeyinde alt ve iç duvar dekompresyonu ile ortalama 5.7 mm, iç alt ve dış duvar dekompresyonu ile ortalama 7 mm, dengeli iç ve dış duvar dekomp-resyonu ile ortalama 4.6 mm, dengeli iç ve dış duvar dekompresyona orbita yağ eksizyonu ek-lendiğinde ortalama 6.5 mm gerileme elde edildi. Olgularda dekompresyon cerrahisi 13 olgunun 20 gözünde kompresif optik nöropati ve ekspojür keratopati gibi fonksiyonel nedenlerle, 22 olgunun 36 gözünde ise kozmetik amaçlı uygulandı. Orbita dekompresyonu sırasında taban eksizyonu yapılan ve yapılmayan olgular cerrahi sonrası yeni gelişen diplopi mevcudiyeti açısından karşılaştırıldığında alt duvar alınarak dekompresyon yapılan olguların, alt duvar eksizyonu uy-gulanmadan dekompresyon yapılan olgulara göre postoperatif yeni diplopi gelişmesi açısından istatistiksel olarak daha fazla risk taşıdıkları tespit edildi (p

Abstract

Purpose: This study reports on the results of current methods of orbital decompression surgery performed in patients with Graves' ophthalmopathy who were operated on for cosmetic and functional purposes. Methods: During the 1994-2001 period, 56 eyes of 35 cases who underwent orbital decompression surgery was evaluated retrospectively. Medial and inferior walls were removed in 9 eyes of 5 patients, medial, inferior and lateral wall in 13 eyes of 7 patients, balanced medial and lateral wall in 18 eyes of 13 patients and balanced medial and lateral wall decompression in conjunction with orbital fat excision was performed in 16 eyes of 10 patients. The incidence of new onset postoperative diplopia in cases with or without inferior wall removal was compared with student t test. Results: The age range of 35 patients (22 female and 13 male) was between 16 and 69 ye-ars (mean 40.4). Follow-up averaged 36.8 months (range 3-72 months) with no patient being lost to follow-up. Medial and inferior wall decompression provided a mean reduction of proptosis of 5.7 mm whereas it was 7 mm with medial, inferior and lateral wall removal, 4.6 mm with balan-ced medial and lateral wall excision, 6.5 mm with balanced medial and lateral wall removal combined with orbital fat excision. In 13 cases (20 eyes) the decompression surgery was performed for functional and in 22 cases (36 eyes) for cosmetic purposes. Those cases who underwent inferior wall removal was found to carry a significantly higher risk of developing postoperative new onset diplopia when compared with cases without inferior wall removal (p
