Özet

Amaç: Kliniğimizde izlediğimiz santral seröz koriyoretinopati (SSR) olgularının demografik, klinik ve anjiografik bulgularını araştırmak ve prognozu etkileyen faktörleri belirleyebilmek. Yöntem: Mart 1992- Mart 2001 yılları arasında klinik ve anjiografik olarak izlenen 56'sı erkek, 6'sı kadın toplam 62 SSR hastasının dosyaları retrospektif olarak incelendi. Başlangıç ve sonuç görme keskinlikleri arasındaki 2 Snellen sırası ve üzerindeki fark ve rekürrens oranı, temel başarı kriteri olarak kabul edildi. SSR'ye neden olan sızıntının tipi ve sızıntı şekli, lokalizasyonu ve sızıntı sayısı, lezyonun foveaya uzaklığı, tedavi uygulanması ve uygulanan tedavinin şeklinin görme artışı ve rekürrens üzerine etkileri araştırıldı. İstatistik yöntemi olarak, Pearson korelasyon analizi kullanıldı. Bulgular: 27 ile 56 yaş arasında yer alan hastaların (ort. 40.3±6.3) başlangıç görme keskinlikleri 0.1 ile 1.0 arasında (ort. 0.62±0.29), sonuç görme keskinlikleri 0.2 ile 1.0 arasında (ort.0.89±0.23) değişmekteydi. Snellen görme eşelinde 2 sıra ve üzeri görme artışı, hastaların %89'unda (n=55) sağlanırken, %11 hastada (n=7) görme artışı izlenmedi. Tedavi uygulanmayan 38 hastanın 36'sında başarı elde edilirken (%94.7), tedavi uygulanan 24 hastanın 19'unda (%79.2) başarı elde edildi. Aradaki fark istatistiksel olarak anlamlı değildi (p=0.098, >0.05). SSR tipi, sızıntı tipi, sızıntı sayısı, sızıntı lokalizasyonu ve koroidal dolaşım bozukluğu ile görme keskinliği artışı ve rekürrens arasındaki ilişki, istatistiksel olarak anlamlı değildi. Sonuç: SSR, ağırlıklı olarak genç ve orta yaş erkeklerde ortaya çıkan, genellikle benign seyirli bir koriyoretinopatidir. Bizim serimizde, görsel başarı açısından, tedavi uygulanan hastalar ile uygulanmayanlar arasında istatistiksel açıdan fark saptanmadı. Bu durumun, yeterli görme keskinliğine belli zamanda ulaşamamış hastalara daha geç dönemde tedavi uygulanması ile ilgili olduğu düşünüldü.

Abstract

Purpose: To evaluate the demographics, clinical and angiographic signs of the centralserous chorioretinopathies (CSC) followed in our department and to designate its prognostic factors. Methods: The clinical files of 62 CSC patients (56 males, 6 females) followed clinically and angiographically between March 1992 and March 2001 were evaluated retrospectivally. The improvement of 2 Snellen lines between the visual acuities of the first and final visits and the rate of recurrence was considered as the basic success criterion. The influence on the visual improvement and the recurrence rate, the type, the manner and the number of the leakage, the distance of the lesion to the fovea, the presence and the type of the therapy were evaluated. Pearson correlation analysis was used as the statistical method. Results: The visual acuity of the patients aged between 27 and 56 years (mean 40.3$pm$6.3) at the first visit was between 0.1 and 1.0 (mean 0.62±0.29) and between 0.2 and 1.0 at the final visit (mean 0.89±0.23). The visual improvement of 2 Snellen lines or more was achieved in 89% of the patients (n=55), no change was seen in 11% of them (n=7). The visual improvement was obtained in 36 of 38 patients of the untreated group (%94.7) and in 19 of 24 patients of the treated group (%79.2). The difference was not statistically significant (p=0.098, (0.05). The relation of the type of the CSC, type, number and the localisation of the CSC and choroidal filling defects with the visual improvement and the recurrence rate was not statistically significant. Discussion: CSC is a benign chorioretinopathy generally seen in young and middle aged men. In our series, the visual improvement in treated patients was not different than untreated patients. This situation may be due to the tendancy of treating the patients with unimproved low visual acuities in the late periods of the desease.
