Fakoemülsifikasyon cerrahisinde oluşan arka kapsül perforasyonu ve zonüler ayrışmanın değerlendirilmesi

Özet

Amaç: Fakoemülsifikasyon cerrahisi sırasında oluşan arka kapsül perforasyonu ve zonüler ayrışmanın, ameliyat sonrası dönemde görme keskinliği ve komplikasyonlar üzerine etkisini araştırmak Gereç ve Yöntem: Mart 1998- Nisan 2002 tarihleri arasında saydam korneal kesiyle fakoemülsifikasyon cerrahisi uygulanan ve ameliyat sırasında arka kapsül perforasyonu ve zonüler ayrışma gelişen 56 hastanın 56 gözü çalışma kapsamına alındı. Olguların yaş ortalaması 62,14±9,7 yıl, ortalama takip süresi 9,2±3,44 ay (2-28 ay) idi. Cerrahi sırasında oluşan arka kapsül perforasyonu ve zonüler ayrışmanın zamanı, kolaylaştırıcı risk faktörleri ve komplikasyonları incelendi. 19 (%33.92) olguda psödoeksfoliasyon, 23 (%41.07) olguda olgun katarakt, 14 (%25) olguda küçük pupilla mevcuttu. Bulgular: 49 (%87.5) gözde arka kapsül perforasyonu, 7 (%12.5) gözde zonüler ayrışma meydana geldi. Arka kapsül perforasyonu, 32 (%65.30) gözde nukleus emülsifikasyonu sırasında, 19 (%38,77) olguda irrigasyon/aspirasyon (I/A) aşamasında, 3 (%6.12) olguda hidrodiseksiyon, ve 2 (%4.08) olguda ise göziçi lensi (GİL) yerleştirilirken oluştu. Arka kapsül perforasyonu gelişen 56 olgunun 51'inde (%91.07) vitreus kaybı meydana geldi ve bunların hepsine aynı seansta ön vitrektomi yapıldı. 14 (%25) göze küçük kesi yerinden kapsül içine göz içi lensi, 14 (%25) göze küçük kesi yerinden sulkusa katlanabilir GIL, 17 (%30.35) göze keşi yeri genişletilerek sulkusa, 5(%8.92) göze aynı seansta skleral tespitli GIL, 3 (%5.35) göze ise başka bir seansta skleral tespitli GIL implantasyonu yapıldı. 3 (%5.35) göz ise afak bırakıldı. Ameliyat sonrası olguların düzeltilmiş en iyi görme keskinliği 39 (%69.64) olguda 5/10 ve üzerindeydi. Düzeltilmiş görme keskinliğini azaltan nedenler arasında en sık kistoid maküler ödem (KMÖ) ve erken dönemde gelişen kornea ödemi mevcuttu. Sonuç: Fakoemülsifikasyon cerrahisinde özellikle eğitim aşamasında uygun hasta seçimi oldukça önemli olmakla birlikte arka kapsül prforasyonu geliştiğinde bile doğru cerrahi yaklaşımla GİL implantasyonu gerçekleştirilip iyi bir görme keskinliği elde edilebilmektedir.

Abstract

Purpose: To evaluate the effect of posterior capsular perforation and zonular tears during phacoemiilsification surgery up on visual acuity and complications. Material and Methods: 6 Eyes of 56 patients that had phacoemiilsification surgery with clear corneal insicion between March 1998 and April 2002 and posterior capsular perforation or zonular tears during the surgery were included in to the study. Mean age of the patients as 62,14$pm$9,7 years old mean follow up time was 9,2±3,44 month. The time of the posterior capsule perforation during surgery and predisposing risk factors and complications were analyzed. There was pseudoexfoliation in 19 (%33,2), mature cataract in 23 (%41,07) patients and small pupil in 14 (%25) patients. Results: There are occured posterior capsule perforation in 49 (%87,5) patients zonuler tear in 7 (%12,5) patients. Posterior capsule perforation occured during phacoemiilsification in 32 (%65,30) eyes, irrigation and aspiration in 19 (%38,77) patients. Hydrodisection in 3 (%6,2) eyes and intraoculer lens implantation in 2 (%3,57). 51 of 56 (%91,07) eyes that had posterior capsule perforation had also vitreus loss we performed anterior vitrectomy to those that had vitreus loss. Through small corneal incision 14 (%25) foldable lOLs are implanted in to capsular bag and 14 (%25) foldable lOLs were implanted in to the sulcus. After enlarging the corneal incision 17 (%30,35) sulcus lOLs were implanted. In the same surgical session 5 (%8,2) scleral fixated lOLs were implanted an in another session 3 (%5,35) scleral fixated lOLs were implanted. 3 (%5,35) eyes were left aphakic. Postoperative best corrected visual acuity were over 5/10 in 39 (%69,64) cases. Cystoid acular edema and early postoperative corneal edema were found to be most common reasons to reduce the visual acuity. Conclusion: In case of posterior capsule perforation with proper surgical approach IOL can be implanted and good visual acuity can be maintained.
