Özet

Amaç: Bu çalışmanın amacı, saydam korneal tünel keşi ile yapılan fakoemülsifikasyon ve katlanabilir göziçi lensi (GİL) İmplantasyonunda enjektör veya forseps kullanımının yara yeriden sızıntıya etkisini araştırmaktır. Gereç ve Yöntem: Fakoemülsifikasyon ile katarakt cerrahisi uygulanan 414 göz çalışma kapsamına alındı. Enjektör ile silikon GİL implantasyonu yapılan gözlerde 3.0 mm'lik kornea kesişi kullanıldı. Diğer tüm olgularda GİL implantasyonu 3.5 mm'lik kornea kesişi ile yapıldı. Ameliyat bitiminde yara dudaklarından humör aköz sızıntısı olup olmadığı kontrol edildi. Sızıntı olan olgulara sütür konuldu. Bulgular: Silikon GİL implante edilen 276 olgunun 100 tanesi (%36,2), akrilik GİL implante edilen 138 olgunun 31 tanesi (%22,4) enjektör kullanılarak implante edildi. Silikon GİL implante edilip, sütür konulan 16 olgunun 5'inde enjektör kullanıldı. Akrilik GİL implante edilip sütür konulan iki olgunun ise bir tanesinde enjektör kullanıldı. Yara yerinden sızıntı ve sütür konulması açısından enjektör ve forseps kullanılan gruplar arasında istatistiksel olarak anlamlı bir fark görülmedi. Sonuç: Sonuç olarak; katlanabilir lens İmplantasyonunda hem forseps hem enjektör sistemlerinin diğer bir deyişle hem 3.0 hem de 3.5mm'lik kornea kesilerinin güvenle kullanılabileceği kanısına varılmıştır.

Abstract

Purpose: The aim of this study is to evaluate the effect of forceps or injector use on wound leak during phacoemulsification and foldable intraocular lens (IOL) implantation through clear corneal tunnel incision.Method: We reviewed a consecutive series of 414 patients who underwent cataract surgery with phacoemulsification. The incision width for IOL implantation was 3.5mm in all cases except eyes which were planned for silicone IOL implantation by injector where incision width was 3.0 mm. Wound leak was controlled at the end of surgery. Patients with wound leak were sutured. Results: Of the 276 patients receiving a silicone IOL, 100 was implanted with injector and of the 138 patients receiving an acrylic IOL, 31 was implanted with injector. Injector was used in five of 16 patients who required suture after silicone IOL implantation and one of 2 patients who required suture after acrylic IOL implantation. There was no statistically significant difference with regard to wound leak and suture requirement between the injector and forceps groups. Conclusion: It is concluded that both forceps and injector use and both 3 and 3.5 mm corneal incision can be used safely during foldable lens implantation.
