The effect of surgical timing on the long-term visual results of patients with natural lens and intraocular lens dislocations

Özet

Amaç: Vitreus veya ön kamaraya disloke olmuş lens veya intraoküler lensleri olan hastalarda tedavi seçeneklerinin ve cerrahi girişim zamanlamasının görsel sonuçlar ile ilişkisinin değerlendirilmesi. Yöntem: Lens veya intraoküler lens lüksasyonu saptanan 29 hastanın 31 gözü retrospektif olarak değerlendirildi. Tedavi yöntemleri olarak pars plana vitrektomi + lensektomi, limbal lens ekstraksiyonu + ön vitrektomi veya konservatif yaklaşım uygulandı. Sonuçlar: Çalışmaya 20 erkek (%69) ve 9 kadın (%31) 29 hastaya ait toplam 31 göz dahil edildi. Olguların yaşları 4 ile 82 (ortalama yaş: 48.6 yıl) arasında değişmekteydi. Dislokasyonlar 10 (%30) gözde katarakt cerrahisi sonucu, 6 (%19) gözde travmaya, 3 (%9) gözde yüksek miyopiye ve l (%3) gözde Marfan Sendromu'na bağlı olarak gelişmişti. 11 (%38) gözde ise herhangi bir neden saptanmadı. Pars plana vitrektomi 20 (%65) göze, limbal lens ekstraksiyonu + ön vitrektomi 7 (%23) göze uygulandı. Girişimde bulunulmayan 4 (%13) gözün ikisinde lens materyali kortikal nitelikteydi, birinde nükleus 1/4'den küçüktü. Bir göz ise hastanın sistemik sorunları nedeniyle konservatif olarak takip edildi. Görmesi değerlendirilebilen 30 gözün 20'sinde (%70) en az l sıra görme keskinliği artışı saptandı, Dislokasyonların tanısı ile cerrahi girişim arasındaki süre ortalama 5.3 hafta olarak belirlendi. İlk 5 hafta içinde cerrahi olarak tedavi edilen 17 olgunun 15'inin (%88), 5 haftadan sonra tedavi edilen 9 olgunun 4'ünün (%44) görme keskinliği düzeyinde artış saptandı. Tartışma: Lens veya intraoküler lens dislokasyonu çoğunlukla cerrahi girişim gerektiren bir klinik durumdur. Bu olgularda erken dönemde cerrahi girişim postoperatif dönemde görme prognozunu olumlu olarak etkilemektedir.

Abstract

Purpose: To evaluate the results of surgical procedures performed for the treatment of natural or intraocular lens dislocations and the effect of surgical timing on the visual outcome.Methods: 31 eyes of 29 patients with natural and intraocular lens dislocations were retros-pectively evaluated. Treatment modalities included pars plana vitrectomy and lens extraction, limbal lens extraction and anterior vitrectomy or conservative treatment. Results: 31 eyes of 29 patients consisting of 20 (69%) males and 9 (31%) females were included in the study. The age of the patients varied between 4 and 82 (mean age: 48.6) years. Ten dislocations (30%) were associated with previous cataract surgery, six (19%) had traumatic dislocations, three (9%) had high myopia and one (3%) had Marfan's Syndrome. No underlying predisposing factor was preseht in 11 (38%) eyes. Pars plana vitrectomy was performed on 20 (65%) eyes and limbal lens extraction and anterior vitrectomy was performed on 7 (23%) eyes. Of the 4 (13%) eyes that were managed conservatively, two had residual cortical material and one had a dislocation of a small nuclear fragment. One patient had systemic problems severe enough to preclude surgery. Of the study group, 20 (70%) eyes had at least one line of gain in visual acuity. The average interval between the onset of dislocation and the surgical procedure was 5.3 weeks. 15 of 17 eyes (88%) operated on before 5 weeks and 4 out of 9 (44%) eyes operated on after 5 weeks had better postoperative visual outcomes. Conclusion: Surgical intervention is nearly always indicated for the treatment of natural or intraocular lens dislocation. Early surgical intervention appears to be associated with a better visual outcome.
