İnfiltratif okuler anesteziden önce lokal soğuk uygulamasının infiltrasyon ağrısına etkisi

Özet
Amaç: Çalışmamızda göz çevresi dokuların soğutulmasının lokal anestezi infiltrasyonu esnasındaki hasta ağrısı üzerine etkisi araştırıldı. Yöntem: Çalışma grubunda retrobulber anestezi uygulanan 36 (yaş ortalaması 42.8±24.2) erkek hasta değerlendirildi. Hastalara katarakt, glokom, hifema lavajı ve erişkin şaşılık operasyonları için van-Lint kapak anestezisi ve retrobulber anestezi uygulandı. Anestezi öncesi retro-bulber anestezi yapılan hastalardan 18'ine, on dakika süre ile silikon jel ile cilt yüzeyinden -4 C° soğuk uygulandı. Anestezi uygulamasının hemen ardından hastaların işlem esnasında duydukları ağrı miktarları sözel olarak (0-4 aralığında) ve görsel analog skala (0-100 aralığında) yardımıyla değerlendirildi, ağrı skorları istatistiksel olarak karşılaştırıldı. Bulgular: Çalışma grubumuz retrobulber anestezi enjeksiyonu esnasında yüzeyel soğutma sonrasında sözel skorlama (2.1±0.9; 1.0±0.4, p=0.00) ve görsel analog skala (48.0±18.7; 33.2$pm$8.4, p=0.02) ile daha az oranda ağrı bildirilirken, soğuk uygulaması ile kapak infiltratif anestezisinde ağrı açısından anlamlı farklılık gözlenmedi (p>0.05). Tartışma: Retrobulber anestezi infiltrasyonu esnasında oluşan ağrıyı azaltmak için lokal soğutmanın etkili olabileceği bulundu.
Abstract
Purpose: In this study the effects of superficial cold aplication on pain during local anest-hetic infiltration was investigated. Methods: In the study group, 36 male patients of different ages (average age of 42.8±24.2 years) were evaluated. Patients were administered van-Lint eyelid anesthesia and retrobulber anesthesia for cataract, glaucoma, hyphema drainage and adult strabismus surgery. Before anesthesia, cold application of -4C ° was performed on 18 patients by the help of a silicon bag for 10 minutes over the skin. On 18 patients, cold application was not performed before anesthesia. Shortly after anesthesia infiltration the pain induced was recorded on a verbal scale (grade 0-4) and visual analogue scale (0-100). Pain scores of patients with and without cold application were compared statistically.Results: Study group with cold application declared less pain during retrobulber anesthesia infiltration with verbal scaling (2.1$pm$0.9; 1.0±0.4, p=0.00) and visual analog scaling (48.0$pm$18.7; 33.2$pm$8.4, p=0.02), however, there was not a significant difference in lid infiltrative anesthesia with cold application. (p>0.05). Conclusion: It was found that superficial cold application was beneficial for reducing pa-in during retrobulber anesthetic application.
