Özet

Amaç: Kanalikülit genellikle tek taraflı, nadir rastlanılan, kolaylıkla atlanabilen ve bu nedenle medikal olarak yeterince tedavi edilemeyen kanalikül enfeksiyonudur. Çalışmamızda amacımız; klasik kanalikülotomiye alternatif tedavi yaklaşımımız olan kanalikül küretajının uygulandığı onüç kanalikülitli olgumuzun sonuçlarını göstermektir. Metod: 1995 ve 2003 yılları arasında, tek taraflı kanalikülit tanısı gösteren onüç olguya önce medikal tedavi uygulandı. Daha sonra sekiz olguya punktoplasti ve kanalikül küretajı yapılarak bütün dakriyolitler çıkarıldı. Peripunktal dokuya 6/0 prolen sütür ile fikse edilmek üzere monokanaliküler silikon stent intübasyonu yapıldı ve 2-4 hafta yerinde bırakıldı. Kalan beş olguya punktoplasti ve silikon stent olmaksızın sadece küretaj uygulandı. Lakrimal pasaj %5'lik povidon iodin solüsyonu ile yıkandı. Çıkarılan dakriyolitler patolojik muayene için gönderildi. Bulgular: Onüç olgunun sekizi kadın ve beşi erkek olup ortalama yaşları 65 ± 13 idi. Bir gözde üst kanalikülit, diğer onikisinde (%92,3) alt kanalikülit tablosu mevcuttu. Daha önce, beş hastaya tek taraflı konjonktivit, beş hastaya da dakriyosistit tanısı ile tedavi uygulanmış olup hastalardan biri de dakriyosistorinostomi ameliyatlısıydı. Diğer üç hastada tipik kanalikülit semptomları vardı. Ortalama semptom süresi 6-18 (10,1±4,2) aydı. Bakteriyel kültür sonuçları yedi hastada pozitif çıktı. Küretaj sonrası iki hastada epifora görüldü ve işlem ikinci kere tekrarlandı. Çıkarılan dakriyolitlerin patolojisinde tüm olgularda Actinomyces Israelii görülürken, beş hastada ilaveten mantar suşları izlendi. Ortalama takip süresi 16,1 ± 6,2 aydı. Olguların hepsinde semptomlar kayboldu, kanalikül normal şekline döndü. Sonuç: Kanalikülit kolaylıkla atlanabilir; bu yüzden kronik konjonktivit ve lakrimal enfeksiyonlu hastalar dikkatli değerlendirilmelidir. Uzun süre antibiyotik ve kortikosteroid kullanılırsa, patoloji sonuçlarında görüldüğü gibi mantarlar üreyebilir. Geniş cerrahi gerekmeksizin basit küretaj ile tüm dakriyolitlerin temizlenmesi hastalığın tedavisi için yeterli olmaktadır. Uygulanan cerrahi metod ne olursa olsun kanalikülden materyalin tamamen çıkarılması için çaba harcanmalıdır; çünki kalan konkresyonlar ve enfekte materyal kanalikülitin kronikleşmesine yol açmaktadır.

Abstract

Purpose : Canaliculitis is a rare, usually unilateral dacryocanal infection that may easily be misdiagnosed, hence not properly treated. We would like to report the results of our treatment approach, which is an alternative to the conventional canaliculotomy on 13 patients with canaliculitis. Methods : Between years 1995-2003, 13 patients with unilateral canaliculitis were first given medical treatment, then 8 patients underwent punctoplasty and curettage of the canaliculus, removing all dacryoliths. A monocanalicular silicon stent intubation was done, the stent fixated to the peripunctal tissue with 6/0 prolene sutures, and left in place for 2 to 4 weeks. The remaining 5 patients had only curettage without punctoplasty and silicone stent. The lacrimal passage was irrigated with 5% povidone iodine solution. The removed dacryoliths were sent for pathology. Results : Of the 13 cases, 8 were females and 5 were males, and mean age was 65±13 SD years. One eye had upper canaliculitis, while the remaining 12(92.3%) had lower canaliculitis, 5 patients with assumed unilateral conjonctivitis and 5 patients with assumed dacryocystitis been previously treated, one also having to have a dacryocystorhinostomy operation. The other 3 patients had typical canaliculitis symptoms. The average symptomatic duration was 6 to 18 (10.1±4.2 SD) months. Bacterial culture results were positive in 7 patients (53.8%). After the curettage, epiphora recurred in two patients, and the procedure had to be repeated. Pathology of the removed dacryoliths showed Actinomyces Israelii in all cases, and 5 cases showed additional fungi. Average follow-up was 16.1±6.2 SD months. In all cases, canaliculitis resolved completely with a patent canaliculus. Conclusion : Canaliculitis can be easily misdiagnosed, therefore should be considered in patients having chronic conjunctivitis or lacrimal infection. The fungi seen on pathology examination was most probably due to long-term antibiotic and corticosteroid therapy. Simple curettage of all dacryoliths without extensive surgery is sufficient for complete resolution. However, whatever the technique for removing the dacryoliths, time should be spent to ensure complete removal of the canalicular contents, because residual concretions or infected material can result in persistent canaliculitis.
