Viskokanalostomi sonrası yüksek moleküler ağırlıklı sodyum hyaluronate bağlı descemet dekolmanı

Özet
Amaç: Glokomun cerrahi tedavisinde yeni bir teknik olan viskokanalostomi ameliyatı sonrası yüksek moleküler ağırlıklı sodyum hyaluronate (Healon GV) kullanımına bağlı olarak geliştiği düşünülen Descemet membranı (DM) dekolmanı olgusu sunulmuştur. Olgu sunumu: 74 yaşında erkek hastanın eksfoliyatif glokomu olan sağ gözüne Stegmann tekniği ile viskonaostomi ameliyatı uygulandı. Cerrahi sırasında ve erken postoperatif devrede komplikasyon görülmedi. Viskokanalostomiden l ay sonra yapılan kontrolde korneanın 1/3 üst nazal kadranında ödem olmaksızın DM dekolmanı tespit edildi. Descemet ayrışması Optik koherens tomografi (OCT) yöntemi ile de gösterildi. Erken postoperatif devrede görme keskinliği etkilenmemekle beraber, kornea ödemi artışı görmede azalma meydana getirdi. Ön kamaraya SF6 gazı verilerek DM dekolmanı yatıştırılamadı. DM dekolmanı alanına parasentez ile girilerek bir viskokanalostomi kanülü yardımı ile yüksek moleküler ağırlıklı sodyum hyaluronate benzer klinik görünümü olan viskoz sıvı aspire edildi sonra 0.3 ml.SFö gazı ön kamaraya verilerek Descemet dekolmanı yatıştırıldı. Bu işlemden sonra korneada eskarlaşma ve endotel yetmezliği gelişmedi. Yatışık Descemet membranı OCT kesitlerinde de gösterildi. Yorum: Descemet membranının trabekulumdan ayrıştırılması esnasında oluşan küçük yırtıkların içine giren viskoelastiğin zaman içinde dekolmana yol açtığı düşünülmüştür. Kornea ödemin nedeni yüksek moleküler ağırlıklı sodyum hyaluronatın bağlı endotel toksisitesi olabilir. Korneada giderek artan ödem meydana geldiği zaman dekole alandan viskoelastik maddenin aspire edilmesi tavsiye edilir.
Abstract
Purpose: There are a few reports about viscocanalostomy complications, which is a new technique in glaucoma surgery. We report a case of Descemet's membrane (DM) detachment after viscocanalostomy, which was possibly due to high-molecular-weight viscoelastic material. Case report: A 74-year-old man with pseudoexfoliative glaucoma in his right eye underwent viscocanalostomy in accordance with Stegmann's technique. No major complication was observed during the operation or the early postoperative period. The IOP decreased gradually after the operation and remained steady without medication during the follow-up period of 18 months. However, a prominent DM detachment localized at 1/3 of the upper nasal quadrant extending to viscocanalostomy site was observed in the first postoperative month. We showed the DM detachment in OCT scans Mild corneal edema appeared with decreasing visual acuity 20 days after the recognition of DM detachment. We made two unsuccessful attempts to reattach the detached area in which we had injected SF6 gas alone into the anterior chamber. During the last surgical intervention, some viscous material was aspirated from the detached area with a Grieshaber cannula and then 0.3 ml of SF6 gas was injected into the anterior chamber. The DM reattached without any major corneal complication except mild wrinkles on Descemet's membrane after the procedure. OCT scans showed reattached DM. Conclusion: In our case, the real mechanism of how the viscoelastic substance detached Descemet's membrane following viscocanalostomy is unclear. High viscosity viscoelastic material might be entering into small ruptures on Descemet membrane during stripping Descemet membrane from trabeculum. It was possible to detach DM due to its gravity in time slowly. The reason of corneal edema may be the toxic effect of viscoelastic material. In our opinion, gentle aspiration of the material combined with SF6 gas injection into anterior chamber was effective.
