Özet

Amaç: Katarakt ameliyatlarında, 5.5 mm optik çaplı 3-parçalı ve tek parçalı hidrofobik akrilik göz içi lensi (GİL) implante edilen hastalarda, arka kapsül kesafeti (AKK) gelişiminin prospektif olarak değerlendirilmesi. Yöntem: Bu çalışma, fakoemülsifikasyon tekniği kullanılarak 4.5-5.0 mm kapsüloreksis ve kapsül içi 5.5 mm optik çaplı 3-parçalı hidrofobik akrilik GİL implante edilen 249 hastanın 267 gözü ile, 5.5 mm optik çaplı tek parçalı hidrofobik akrilik GİL implante edilen 241 hastanın 252 gözünü kapsamaktadır. Toplam 493 hastanın 519 gözü 18 aylık dönemde arka kapsül kesafeti gelişimi açısından prospektif olarak değerlendirildi. Bütün hastalar, periodik ön segment retroillüminasyon fotoğrafları çekilerek analiz edildi. Elde edilen veriler ki-kare yöntemi ile değerlendirildi. Sonuçlar: Tek parçalı ve 3-parçalı hidrofobik akrilik GİL implantasyonu arasında arka kapsül kesifliği yönünden istatistiksel olarak anlamlı bir fark bulunmadı. Ancak tek parçalı hidrofobik akrilik GİL implante edilen grupda, haptik-optik birleşim yerindeki arka kapsül bölgesinde daha belirgin kesafet olması dikkat çekiciydi. İzlem süreci boyunca 5.5 mm optikli hidrofobik akrilik GİL implante edilen hastaların tümünde Neodymium:YAG (Nd:YAG) lazer kapsülotomi gerektirmeyecek derecede AKK görüldü. Tartışma: Biyouyumluluk ve azalmış AKK son jenerasyon GİL'lerin özellikleri arasında başta gelmektedir. Tek parçalı ve üç parçalı hidrofobik akrilik GİL'lerin, postoperatif 18 aylık dönemde Nd: YAĞ laser kapsülotomi gerektirmemeleri nedeniyle katarakt cerrahisinde tercih edilmelerinin hasta ve hekim beklentilerine olumlu etkileri olacaktır.

Abstract

Purpose : To evaluate the development of posterior capsule opacification (PCO) in patients implanted with 5.5 mm optics single piece and 3-piece hydrophobic acrylic intraocular lens (IOL) in cataract surgery prospectively. Methods : This study, was carried on 267 eyes of 249 patients implanted with 3-piece 5.5 mm optics hydrophobic acrylic intraocular lens (IOL) and 252 eyes of 241 patients implanted with single piece 5.5 mm optics hydrophobic acrylic IOL by phacoemülsification technique, 519 eyes of 493 patients were prospectively evaluated for the PCO development in the period of 18 months. All the patients were analyzed periodically with anterior segment retroillumination photography. The data provided were analyzed with chi-square method. Results : There was no statistically significant results between the two groups for PCO development. But, it should be underlined that there was a prominent opacification on posterior capsule where the optic and haptic junctions corresponded in some patients implanted with single piece IOL. During the follow-up period, no patients implanted with either 3-piece or single piece hydrophobic acrylic IOL necessitated Nd-YAG laser posterior capsulotomy. Conclusion : Biocompatibility and reduced rate of PCO development are among the leading features of new generation IOL´s. The unnecessity of Nd:YAG laser capsulotomy requirment of single piece and 3-piece hydrophobic acrylic IOL during the 18 month period is the reason for the preferance in cataract surgery and has a positive effect on both the patient and surgeon expectancy.
