Özet

Amaç: Nd:YAG laser ile yapılan arka kapsülotomi girişimlerinin kırma kusuru ve ön segment biyometri bulgularına etkisinin incelenmesi. Gereç ve Yöntem: Fakoemülsifikasyon veya kombine fakoemülsifikasyon-trabekülektomi + göz içi lens uygulanan ve sistemik problemi olmayan hastalardan arka kapsül kesafeti gelişenler çalışmaya alındı. Tüm olguların tam göz muayeneleri gerçekleştirildi. Olgulara arka laser kapsülotomi uygulanacağı gün ve işlemden sonraki 1. ve 3. aylarda; en iyi düzeltilmiş görme keskinliği, kırma kusuru, göziçi basınç, keratometri ve ön kamara derinliği ölçümleri birer kez gerçekleştirildi. Çalışmanın istatistiksel analizinde; tekrarlı ölçümlerde varyans analizi, Maun Whitney U testi ve tanımlayıcı istatistikler kullanıldı. P değerinin 0.05'in altında olması istatistiksel olarak anlamlı kabul edildi. Bulgular: Çalışmaya 39'u kadın, 48'i erkek olan 87 olgunun toplam 93 gözü dahil edildi. Olguların ortalama yaşı 51,8 ± 9,3 yıl (min-maks, 34-72 yıl) olarak saptandı. Katarakt cerrahisi ile laser kapsülotomi arasındaki süre ortalaması 15,4 ± 3,8 ay (min-maks, 3- 46 ay) olarak saptandı. Tüm gözlerin görme keskinliklerinde laser kapsülotomi sonrasında anlamlı artışlar izlendi. Laser kapsülotomi sonrası 1. ayda tüm olguların ön kamara derinlikleri, kırma kusurları ve keratometri değerlerinde anlamlı farklılıklar saptandı.Üçüncü ayda yapılan ölçümlerde, birinci ay sonuçlarından anlamlı farklılık bulunmadı (p>0,05). Göziçi basınçlarında, laser kapsülotomi sonrası anlamlı değişiklikler saptanmadı (p>0,05). Sonuç: Nd: YAĞ laser kapsülotomi, görsel rehabilitasyonu sağlamada etkili ve güvenilir bir işlemdir. Laser kapsülotomi sonrası hastalarda gerekebilecek gözlük tahsis muayeneleri için, girişim sonrası bir ay beklemek yeterli görünmektedir.

Abstract

Purpose : To evaluate the effect of Nd:YAG laser posterior capsulotomy on the refractive error and the anterior segment biometry findings. Methods : Those patients, with no systemic problems, who developed posterior capsule opacification after the phacoemulsification or phaco-trabeculectomy + intraocular lens implantation were included in the study. Complete ocular examinations were performed for each patient. The best corrected visual acuity, refractive error, intraocular pressure, keratometry and the anterior chamber depth measurements were obtained on the day of procedure, and 1 and 3 months post-laser, respectively. Repeated measures of ANOVA, Mann Whitney U test and the descriptive statistics were used for the statistical analysis of the study. P value less than 0.05 were considered to be significant statistically. Results : A total of 93 eyes of 87 subjects (39 females and 48 males) were included in the study. The mean age of the subjects was 51.8 ± 9.3 years (ranged, 34 to72 years). The mean duration between the cataract extraction and the capsulotomy was 15.4 ± 3.8 months (ranged, 3 to 46 months). There was significant improvement in the visual acuity of all eyes after capsulotomy. Significant differences were found in the anterior chamber depth, refractive error and the keratometry readings in the post-laser 1- month examination. There were no statistically significant differences between the refractive error and the biometric measurements obtained at the post-laser 1st- and the 3rd- month exams (p>0.05). There were no significant changes in the intraocular pressure readings upon capsulotomy (p>0.05). Conclusion : Nd: YAG laser capsulotomy is an effective and a safe procedure for improving visual rehabilitation. It seems enough to wait for only one month after the laser posterior capsulotomy for the probable spectacle examination of the patients in case they require.
