Özet

Amaç: Vogt-Koyanagi-Harada sendromu tanısı alan 3 hastada farklı klinik seyirlerin ve pulse kortikosteroid tedavisinin etkinliğinin değerlendirilmesi.Yöntem: 1993-2003 yılları arasında bilateral eksudatif retina dekolmanı ile üvea polikliğinde takibe alınan Vogt-Koyanagi-Harada sendromlu 3 olgu retrospektif olarak incelenmiştir. Bütün hastalara, 3 gün süre ile 1 gr/gün pulse metilprednizolon sonrası, oral idameye geçilmiştir. 3. hastaya üç kez pulse steroid tedavisini takiben siklosporin A ve azathioprine de uygulanmıştır. Hastalar düzenli olarak görme keskinliği, retina muayenesi, fundus fiöresein anjiografisi ve fundus fotoğrafları ile izlenmişlerdir. Bulgular: Yaşları 17 ile 42 arasındaki 3 hasta çalışmaya alındı. Her üç hasta da el hareketi düzeyinde görme keskinliği ile başvurmuştur. Pulse kortikosteroid tedavisinden sonra 1. hastada 6 ay içerisinde görme keskinliği her iki gözde 0.8 düzeyine ulaşmış, altı yıllık ilaçsız takibinde rekürrens gözlenmemiştir. 2. hastanın iki ay içerisinde görme keskinliği sağ gözde 0.7, solda 0.9 seviyesine ulaşmış, ilaçsız ikinci ay takibinde retina dekolmanı nüksü ile görme keskinliği parmak sayma düzeyine inmiştir. 2. pulse kortikosteroid tedavisinin ardından eksudatif retina dekolmanı gerilemiş, görme keskinliği sağ gözde 0.5, sol gözde 0.4 seviyesinde kalmıştır. 3. hasta optik diski de içine alan total eksudatif dekolmanla başvurmuş, 3 kez pulse kortikosteroid tedavisine ek olarak siklosporin A ve azathioprine tedavisi uygulandığı halde subretinal bandları olan optik disk ve çevresi dışındaki dekolman alanı yatışmış, görme keskinliğinde artış olmamıştır. Bütün hastalarda eski dekolman alanında korioretinal atrofi ve pigmente lezyonlar gelişmiştir.Yorum: Vogt-Koyanagi-Harada sendromu yakın izlem gerektiren ağır göz tutulumları ile karşımıza çıkmaktadır. Vizüel prognozu, retina tutulum derecesi ve medikal tedaviye yanıt belirlemekte, bu da olgudan olguya farklılıklar gösterebilmektedir.

Abstract

Purpose: To evaluate the different clinical courses and the effect of pulsed steroid treatment in 3 patients diagnosed as Vogt Koyanagi Harada (VKH) syndrome. Materials and Methods: Clinical reports of 3 patients with VKH syndrome all followed in uvea clinic from 1993 to 2003 were evaluated retrospectively. All patients were treated with 1 g/day pulsed methylprednisolone for 3 days followed by oral corticosteroid therapy. Cyclospo-rine A and azathioprine therapy was applied to the third patient following pulsed steroid treatment. Regular follow-up was done with visual acuities, retinal examination, fundus fluorescein angiography and fundus photographs. Results: Study group consisted of 3 patients aged between 17-42 years. Each of the three patients admitted to the hospital with a visual acuity level of hand motions. After pulsed corticosteroid treatment, the visual acuity of the first patient reached to 8/10 in both eyes within six months and no recurrence has been observed for six years of follow-up. the visual acuity of the first patient reached to 8/10 in both eyes within six months and no recurrence has been observed for six years of follow-up. The second patient's visual acuity reached to 7/10 OD and 9/10 OS within two months. During the follow-up period of two months without medication, visual acuities decreased to counting finger level due to recurrence of retinal detachment. After second pulsed corticosteroid treatment, retinal detachment regressed with remaining visual acuities of 5/10 OD and 4/10 OS. The third patient presented with bilateral total exudative retinal detachment including the optic disks; in spite of pulsed corticosteroid treatment, cyclosporine A and azathioprine therapy, no increase in visual acuity has occurred yet though retinal detachment except the peripapillary area with subretinal fibrotic bands regressed. In all patients, chorioretinal atrophy and pigmented lesions developed in the previous detachment area.Interpretation: Vogt Koyanagi Harada syndrome causes extensive eye involvement that requires close follow-up. The visual prognosis depends on the degree of retinal involvement and response to medical treatment which can show differences from case to case.
