Özet

Amaç: Retinitis pigmentoza hastalığı ve spontan lens rezorpsiyonu olan iki kardeşin tanımlanması.Gereç ve Yöntem: Her iki gözde ilerleyici gece görme kaybı ve sağ gözde ciddi görme azlığı şikayeti ile başvuran 25 yaşındaki erkek hastanın soy geçmişinde anne babasında birinci derece akraba evliliği olduğu, benzer şikayeti olan iki erkek kardeşi bulunduğu, üç kız kardeşinin ilk iki yaş içinde exitus olduğu öğrenildi. Hastaya ve etkilenmiş erkek kardeşlerinden birine tam oftalmolojik muayene, fundus fluorescein anjiografi, ultrasonografi ve elektrofizyolojik tetkikler yapıldı. Bulgular: Yapılan muayenede, hastanın görme keskinliği sağda ışık persepsiyonu, solda ise 0.05 düzeyinde saptanmıştı. Ön segment muayenesinde sağ gözde lens kapsülü dışında lens materyelinin izlenmediği, lens kapsülünün kesif olduğu ve seklüzyo pupilla varlığı tespit edildi. Solda mikrosferofaki ile beraber kortikonükleer kesafet mevcuttu. Fundus muayenesinde, sağda fundus aydınlanmıyordu, soldaysa arterlerde incelme, optik atrofi, kemik spikülü tarzında pigmentasyon ve makulada kistoid makula ödemi sekeli izlenmekteydi. A-scan ultrasonografide sağ ve sol göz aksial uzunlukları normaldi. Elektrofizyolojik incelemesinde amplitudları ölçülemeyecek kadar düşük izlenmekteydi. Hastanın sol gözüne fakoemülsifikasyon ile katarakt ekstraksiyonu uygulanmasını takiben görme keskinliği 0.2'ye yükseldi. Sağ göze uygulanan YAG-kapsülotomiye rağmen görme keskinliğinde artış izlenmedi. Hastanın muayene olan erkek kardeşinin sağ gözü absolu iken sol gözde görme keskinliği el hareketi düzeyindeydi. Oftalmolojik muayenesinde her iki gözde kalsifie lens bakiyeleri ve opak lens kapsülü, sol gözde kardeşine benzer fundus bulguları saptanmıştı. Elektrofizyolojik incelemesinde ve A-scan ultrasonografi-sinde kardeşine benzer bulgular saptanmıştı. Sonuç: Literatürde retinitis pigmentoza ile birlikte bildirilen lens patolojileri özellikle arka subkapsüler katarakt ile sınırlıdır. Olgumuz ve kardeşi retinitis pigmentoza ile birlikte spontan lens rezorpsiyonu -yani travma ya da cerrahi olmaksızın lensin normal bütünlüğünün kaybedilmesi- görülen ilk vakalardır.

Abstract

Purpose: Definition of two brothers with retinitis pigmentosa and spontaneous lens resorption. Methods: A 25 year old male patient with the complaint of progressive decreased night vision in both eyes and severely decreased vision in the right eye had been accepted at our outpatient clinic. In his pedigree analysis it was noticed that his mother and father were first degreee relatives, he had two brothers affected like him and three sisters who had been exitus at about 2 years of age. The patient and one of his affected brothers had full ophthalmological examination, fundus fluorescein angiography, ultrasonography and electrophysiological tests. Results: His visual acuities were light perception and 0.05 on the right and left eyes respectively. On anterior segment examination lens material could not be seen other than the opa-sified lens capsule and secclusio pupilla was observed on the right side. On the left eye, there was microspherophakia and corticonucleer cataract. During fundus examination, the right fundus was not visible while arterial narrowing, optic atrophy, bone spicule pigmentation and sequels of cystoid macular edema were viewed on the left eye. With A-scan ultrasonography the axial diameters of both eyes were normal. At electrophysiology the amplitudes were so low that they could not be measured. After removing his lens material with cataract by phakoemulsifica-tion his visual acuity increased to 0.2 on the left eye. The acuity on the right eye did not improve after applying YAG capsulotomy. The brother examined could see only hand motion with the left eye while the right eye had no light perception. On ophthalmological examination in both eyes lens material could not be seen other than the opasified lens capsule and fundus findings of the left eye were identical to those of his brother. With A-scan ultrasonography and electrophysiology the findings were similar to those of his brother. Conclusion: In the literature lens pathologies seen in combination with retinitis pigmentosa are limited especially with posterior subcapsular cataract. To our knowledge our patient and his brother are the first retinitis pigmentosa cases with spontaneous lens resorption that is the dissolution of normal lens without a history of trauma or surgery.
