Tedaviye dirençli oküler yüzey patolojlerinin tedavisinde amniyon zarı implantasyonu sonuçlarımız

Özet
Amaç: Tedaviye dirençli oküler yüzey patolojisi nedeniyle uyguladığımız amnion zarı transplantasyonu sonuçlarımızın değerlendirilmesi Gereç ve Yöntem: Kasım 2001 ve Mart 2004 ayları arasında; steril stromal ülser (5 göz), keratoplasti sonrası melting / persistan epitel defekti (4 göz), herpetik keratit sekeli (2 göz), bakteriyel korneal abse (2 göz), sekonder IOL implantasyonu sonrası sklera defekti (2 göz), korneal perforasyon (2 göz), büllöz keratopati (1 göz), travma (2 göz), trahom sekeli (1 göz), katarakt operasyonu persistan epitel defekti (1 göz) ve kireç yanığı sonrası persistan epitel defekti (1 göz) nedeniyle hastanemizde amnion zarı transplantasyonu (AZT) yapılan toplam 23 göz retrospektif olarak incelendi. Amnion zarı, vakalarin hepsinde korneal lezyonun yeri ve boyutundan bağımsız olarak turn korneayi kaplayacak şekilde Örtüldü ve perilimbal konjonktivaya 10/0 monofilaman sutur ile 360 derece suture edildi. Operasyon sonunda terapötik kontakt lens yerleştirildi. Çalışma kapsamına alınan vakalarda yüzey epitel bütünlüğünün oluşma süresi, nüks varlığı ve görme keskinliği değerlendirildi. Bulgular: Amniyon zar transplantasyonu sonrası ortalama 10.45 ± 15.81 ay takip edilen vakalarda, oküler yüzey patolojisinin ortalama iyileşme süresi 5.48 ± 5.83 hafta idi. 16 gözde (%69.6) 1 kere, 4 gözde (%17.4) 2 kere, 3 gözde (%13) 3 kere transplantasyon gerekli oldu, ve postoperatif 10 hastaya (%43.48) penetran keratoplasti (PKP) önerildi, 1 gözde (%4.17) limbal kök hücre transplantasyonu ve superfisiyal keratektomi uygulandı. Preoperatif en iyi düzeltilmiş görme keskinliği ortalama 0.06 ±0.13 iken, postoperatif son kontrolde 0.16 ± 0.28 ye yükseldi (p=0.04). Toplam 23 gözden 16sında (%69,57) postoperatif en iyi düzeltilmiş görme keskinliğinde artış oldu. Sonuç: Tedaviye dirençli oküler yüzey patolojilerinde, insan amnion zari ile oküler yüzeyin örtülmesinin yararlı bir tedavi metodu olduğu kanısına varıldı.
Abstract
Purpose: Evaluation of the results of amnion membrane transplantation procedure for the treatment of ocular surface pathology. Setting: Beyoğlu Eye Education and Research Hospital Material and Method: A total of 23 eyes who were performed amnion membrane transplantation in our clinics between November 2001 and March 2004; for the treatment of sterile stromal ulcer (5 eyes), melting /persistant epithelial defect following keratoplasty (4 eyes), herpetic keratitis sequela (2 eyes), bacterial corneal abscess (2 eyes), scleral defect following secondary IOL implantation (2 eyes), corneal perforation (2 eyes), bullous keratopathy (1 eye), trauma (2 eyes) trahoma sequela (1 eye), persistant epithelial defect following cataract operation (1 eye) and persistant epitheliel defect following lime burn (1 eye); were examined retrospectively. Amnion membrane was covered all over the cornea independent of the position and size of the lesion and was sutured on the perilimbal conjunctiva circumferentially by 10/0 monofilament sutur. At the end of the operation, therapeutic contact lens was put. The cases included in the study were evaluated with respect to duration of surface epithelial healing, presence of recurrence and visual acuity. Results: Following the amnion membrane transplantation, cases were followed up for a mean of 10.45 ± 15.81 months, ant the mean healing time was 5.48 ± 5.83 weeks. Amnion membrane transplantation was performed once in 16 eyes (69,6%), twice in 4 eyes (17.4%), three times in 3 eyes (13%); limbal stem cell transplantation and superficial keratectomy were performed in 1 eye (4,17%). At the end of the follow up, penetrating keratoplasty was recommended for 10 eyes (43,48%). Preoperatively mean best corrected visual acuity (BCVA) was 0.06 ±0.13, while in the postoperative last follow-up it was 0.16 ± 0.28 (p=0.04). At the end of the follow up, BCVA was improved in 16 of the 23 eyes (%69,57). Conclusion: Ocular surface reconstruction with human amnion membrane is thought to be a useful treatment modality, in ocular surface disorders refractory to the treatment.
