Küçük pupillası ve kataraktı olan olgularda basit germe teknikleri ile fleksibl iris retraktörlerinin karşılaştırılması

Özet
Amaç: Küçük pupilla ile birlikte kataraktı olan olgularda basit germe teknikleri ile iris retraktörlerinin etkinliğinin ve komplikasyonlarının karşılaştırılması. Metod: Küçük pupilla ve katarakt nedeni ile fakoemülsifikasyon ile birlikte göz içi lens implantasyonu uygulanan 22 olgunun 30 gözü çalışma kapsamına alındı ve olgular her bir grupta 15 göz olacak şekilde iki eşit gruba ayrıldı. Grup 1'de küçük pupillayı genişletmek amacı ile basit germe teknikleri, grup 2'de ise fleksibl iris retraktörleri uygulandı. Tüm olgularda cerrahi öncesi ve sonrası görme keskinliği, göz içi basınç (GİB) ölçümleri, ön segment bulguları, ameliyat esnasında ve sonrasında gözlenen komplikasyonlar kaydedildi. Bulgular: Ameliyat öncesi gruplar arasında yaş, cinsiyet ve tanı açısından istatistiksel anlamlı farklılık yoktu (P>0.05). Ameliyat esnasında grup 1 ve grup 2'de en sık gözlenen komplikasyon, pigment salınımı idi. Ameliyat sonrası erken dönemde ön kamara reaksiyonu oranı grup 1'de grup 2'e oranla istatistiksel olarak daha yüksek olarak saptandı. Ameliyat sonrası geç dönemde ise arka sinesi ve arka kapsül kesafeti oranlan grup 1'de istatistiksel olarak anlamlı olmamakla birlikte yüksek idi. Sonuç: Fleksibl iris retraktörleri kullanımı, küçük pupillalı gözlerde basit germe yöntemlerine göre fakoemülsifikasyon cerrahisi esnasında daha fazla pupilla genişliği sağlaması, travmayı en aza indirmesi ve postoperatif dönemde oluşabilecek komplikasyonları azaltması nedeni ile tercih edilebilecek bir yöntemdir.
Abstract
Purpose: To compare the efficacies and complications of simple stretching techniques and flexible iris retractors in patients with small pupil and cataract. Methods: Thirty eyes of 22 patients with small pupil and cataract who had undergone phacoemulsification with intraocular lens implantation were included in the study and the patients were equally (in each 15 eyes) divided into two groups. To widen small pupil, simple stretching methods were used in group 1 eyes and flexible iris retractors in group 2 eyes. In all patients, visual acuities, intraocular pressures (IOP), anterior segment findings before and after surgery, and intra-and postoperative complications were all recorded. Results: There were no statistically significant differences in age, sex and preoperative diagnosis between two groups (P>0.05). The most common intraoperative complication was pigment releases in both groups. In the early postoperative period, anterior chamber reaction was significantly higher in group 1 when compared with group 2. In the late postoperative period, the rates of posterior synechia and posterior capsule opacification were higher in group 1, but there were no statistically significant differences. Conclusion: The use of flexible iris retractors may be a preferable method in eyes with small pupil by providing wider pupil during phacoemulsification surgery, minimizing surgical trauma and decreasing the rate of postoperative complications when compared with simple stretching methods.
