Özet

Amaç: Beyaz kataraktlarda, fakoemülsifikasyon cerrahisi sırasında ön kapsül boyası olarak tripan mavisi kullanımının kornea kalınlığı ve ön kamara reaksiyonu üzerine etkisini araştırmak.Yöntem: Kapsüloreksis öncesinde tripan mavisi kullanılan, matür ve entümesan kataraktlı 26 olgunun 26 gözü (Grup 1) ile çeşitli tipte kataraktı bulunan ve tripan mavisi uygulanmayan 26 olgunun 26 gözü (Grup 2) çalışma kapsamına alındı. Kornea kalınlıkları Sonomed A-Scan ultrasonik pakimetre cihazı ile operasyon öncesi, operasyon sonrası l.gün, 1.hafta, l.ay, 3.ay ve 6.ayda ölçüldü. Santral ve periferik kornea kalınlıkları karşılaştırıldı, istatiksel yöntem olarak Student t-testi kullanıldı. Aynı aralıklarla ön kamaradaki reaksiyon postoperatif olarak değerlendirildi, istatiksel yöntem olarak Fisher testi kullanıldı. Bulgular: Her iki grubun operasyon öncesi santral (p=0,37) ve periferik (p=0,21) kornea kalınlıkları ve fakoemülsifikasyon süreleri (p=0,17) arasında anlamlı fark yoktu. Postoperatif l.gün Grup 1'in santral kornea kalınlığı 714±123 mikron, periferik kornea kalınlığı 858±104 mikron, Grup 2'de ise santral kornea kalınlığı 629±111 mikron, periferik kornea kalınlığı ise 787±99 mikron idi. Postoperatif 1.günde Grup 1'in ortalama santral ve periferik kornea kalınlıkları Grup 2'nin santral (p=0,02) ve periferik (p=0,02) kornea kalınlıklarından istatiksel olarak anlamlı derecede fazlaydı. Postoperatif takip eden dönemlerde iki grup arasında kornea kalınlıkları açısından anlamlı bir fark bulunmadı. 1.aydan itibaren kornea kalınlıkları her iki grupta preoperatif değerlerine ulaştı. Postoperatif ön kamara reaksiyonu değerlendirilmeleri açısından iki grup arasında anlamlı bir fark bulunmadı (l.gün p=0,7, diğer günler p=l,0). Sonuç: Beyaz kataraktlarda kapsüloreksis öncesinde tripan mavisi kullanımının kapsüloreksis yöntemini kolaylaştırdığı, postoperatif erken dönem dışında kornea kalınlığı ve ön kamara reaksiyonu üzerine anlamlı etkisi olmadığı görüldü.

Abstract

Purpose: To evaluate the effects of trypan blue, as a capsule paint, on the corneal thickness and anterior chamber reaction, during phacoemulsification of white cataracts. Methods: 26 eyes of 26 patients with mature and entumescent cataract, on whom trypan blue was used before capsulorhexis (Group 1) and 26 eyes of 26 patients with different types of cataracts, on whom trypan blue was not used, (Group 2) were included in the study. Corneal thickness was measured with Sonomed A-Scan ultrasonic pachymetry before the operation, first day, first week, first month, third month and the sixth month. Thickness of central and peripheral cornea were compared with the statistical method of Student t-test. With the same time intervals, the anterior chamber reactions in the two groups were also examined postoperatively and compared with the Fishers' exact test. Results: There was no statistically significant difference between the central (p=0,37) and peripheral (p=0,21) corneal thicknesses preoperatively and phacoemulsification time (p=0,17). On the first postoperative day, the central corneal thickness of Group 1 was 7141123 micron, the peripheral corneal thickness was 858±104 micron. The central corneal thickness of Group 2 was 629±111 micron, the peripheral corneal thickness was 787±99 micron. On the first postoperative day, the mean central and peripheral corneal thicknesses of Group 1 were statistically significantly higher than the mean central (p=0,02) and peripheral (p=0,02) corneal thicknesses of Group 2. On the following examination periods there was no difference statistically between the two groups according to the corneal thicknesses. Corneal thicknesses reached their preoperative values 1 month after operation. Postoperative anterior chamber reaction examinations between the two groups were also insignificant. Conclusion: Using trypan blue in the eyes of patients with white cataract, facilitates the capsulorhexis. It has no significant effect on the corneal thickness, except on the early postoperative days, and anterior chamber reaction.
