Şaşılık ameliyatı sonrası oluşan ölüm olgusunun deontolojik ve tıbbi etik açıdan irdelenmesi

Özet
Amaç: Ölüm ile sonuçlanmış ve yargıya intikal etmiş bir şaşılık ameliyatının aşamalarını deontolojik ve etik açıdan irdelemek.Olgu: 3 yaşındaki kız çocuğu diverjan strabismus nedeniyle genel anestezi altında ameliyat edilmek üzere hazırlanmıştır. Anamnezinde havale saptanan olguya ameliyat öncesi bazı rutin laboratuvar tetkikleri yapılmıştır. Ameliyat normal seyretmiş, ameliyat bitiminde hasta uyanamamış, yüksek ateş ve taşikardi gelişmiş, malign hipertermi nedeniyle eks olduğu rapor edilmiştir.Yorum: Yargı tarafından cerrahi sorumluluğun ikinci evresi olan ameliyat hazırlığı ve dördüncü evresi olan ameliyat sonrası bakım evrelerinde dikkatsizlik ve tedbirsizlik sonucu ölüm oluştuğu hükmüne varılmıştır.İndikasyonu koymakla başlayan cerrahi sorumluluk, müdahaleden sonra da devam eder. Cerrah, hasta narkozdan çıkıncaya kadar süren sorumluluğuna ayrıca, ameliyat sonu teknik bakımın da ekli olduğunu bilmelidir. Bu olguda cerraha yüklenen kusur oranı bu aşamalar nedeniyle verilmiştir.
Abstract
Objective: To evaluate a case of death after strabismus surgery in the perspective of medical deontology and medical ethics. Case: 3 year-old girl had been operated for divergent strabismus under general anesthesia. In her story, she had convulsion. After the operation she could not get over the anesthesia period. The cause of exitus was reported as she died due to malignant hyperthermia. Comment: The surgical responsibility begins with the indication stage. The second stage of surgical responsibility is preparation to the surgery, the third is the operation, and the fourth is postoperative medical and technical care. In this case, both of the doctors, the anesthesiologist and the ophthalmologist had been blamed because of insufficient preoperative preparation and postoperative care. In surgical interventions which arc under anesthesia, the surgeon must be aware of responsibility with the anesthesiologist in all stages of the operation.
