Özet

Amaç: İntravitreal triamsinolon asetonid (IVTA) sonrası gelişen endoftalmi insidansının belirlenmesi, klinik seyir ve sonuçların değerlendirilmesi. Metod: Değişik nedenlere bağlı IVTA uygulanan 190 olgunun 245 gözü çalışma kapsamına alındı. Tüm olgularda 8 mg/0.2 mi IVTA enjeksiyonu öncesi ve sonrası slit-lamp biomikroskopi, göz içi basıncı (GİB) ölçümü ve fundus muayenesini içeren tam bir oftalmolojik muayene yapıldı. Bulgular: Ortalama takip süresi 11.2±6.3 ay arasında değişmekte olup sadece bir olguda IVTA sonrası akut endoftalmi saptandı (1/305 enjeksiyon, %0.32). Bu olgu, 27 yaşında erkek hasta olup Behçet hastalığına bağlı arka segment tutulumu var idi. IVTA sonrası 6. günde endoftalmi tanısı konuldu ve mikrobiolojik incelemede koagülaz negatif Stafilokoklar tespit edildi. Yoğun topikal, subkonjonktival, sistemik ve intravitreal antibiotik ve kortikosteroid tedavisi uygulanmasına rağmen olgunun görme keskinliği ışık persepsiyon seviyesine düştü. Bunun üzerine olguya pars plana vitrektomi uygulandı ve cerrahi sonrası 2. ayda olgunun görme keskinliği 20/400'a yükseldi. Sonuç: IVTA uygulaması sonrası endoftalmi riski mevcut olup bu tür olguların sıkı bir şekilde takibi gereklidir.

Abstract

Purpose: To determine the incidence of endophthalmitis after intravitreal triamcinolone acetonide (IVTA) injection and to evaluate the clinical progress and results. Materials-Methods: Two hundred forty-five eyes of 190 patients who had undergone IVTA injection because of various reasons were included in the study. In all patients, a complete ophthalmological examination including slit-lamp biomicroscopy, intraocular pressure (IOP) measurement and fundus examination was performed before and after 8 mg/0.2 ml injection of triamcinolone acetonide, intravitreally. Results: The mean follow-up time was 11.2±6.3 months, and only one case of acute endophthalmitis was encountered after IVTA injection (1/305 injection, 0.32%). The patient was a 27-years old man and had Behcet's disease with posterior segment involvement. Six days after IVTA injection, he was diagnosed as endophthalmitis, and microbiological examination revealed caogulase-negative Staphylococcus. Although intensive topical, subconjunctival, systemic and intravitreal antibiotic and corticosteroid were applied, the visual acuity of this patient dropped to light perception. The patient underwent pars plana vitrectomy, the visual acuity at 2-month after surgery improved to 20/400. Conclusion: IVTA injection may be associated with a risk of acute endophthalmitis; therefore, such patients should be closely followed up.
