Preklinik psödoeksfoliasyon ile psödoeksfoliatif sendromlu olguların klinik ve cerrahi yönden benzerlikleri

Özet

Amaç: Ön segment muayenesinde psödoeksfoliasyon materyali saptanmayan ancak şüpheli klinik bulguları olan "Preklinik psödoeksfoliasyon"lu olgular ile Psödoeksfoliatif sendrom (PES)'lu olguları klinik bulgular ve intraoperatif komplikasyonlar açısından kontrol grubuyla karşılaştırmak.Yöntem: Şişli Etfal Eğitim ve Araştırma Hastanesi Göz Kliniği'nde Mayıs 2001 ve Ağustos 2003 tarihleri arasında, en az bir gözü katarakt cerrahisi veya trabekülektomi ile kombine katarakt cerrahisi için yatırılan, 50 yaş ve üzeri, 905 olgunun 1244 senil kataraktlı gözü retrospektif olarak değerlendirildi. Olgular ön segment bulguları değerlendirilerek üç gruba ayrıldı. Grup I Psödoeksfoliatif sendrom, Grup II preklinik veya şüpheli psödoeksfoliasyon ve Grup III kontrol grubundan oluşmaktaydı. Bu üç grup yaş, cinsiyet, sistemik hastalık, ön segment bulguları ve özellikle cerrahi esnasında gelişen komplikasyonlar açısından karşılaştırıldı. Grup I ve Grup II arasındaki benzer ve farklı yönler irdelendi. Bulgular: PES ve şüpheli-preklinik psödoeksfoliasyon arasında yaş, cinsiyet, sistemik hastalık, görme düzeyi, tansiyon oküler ve katarakt tipi arasında anlamlı fark saptanmamıştır. Grup I ve grup II arka kapsül perforasyonu ve zonül dializi açısından değerlendirildiğinde istatistiksel anlamlı fark saptanmadı (p>0,05). Abstract

Purpose: To compare cases with "Preclinic-masked pseudoexfoliation" that have suspected clinical findings but have no pseudoexfoliative material in their anterior chamber and Pseudo exfoliative syndrome (PES) cases with control group according to clinical findings and intraoperative complications. Methods: 1244 eyes of 905 cases operated on in Ophthalmological Department of Şişli Et-fal Education and Research Hospital with senile cataract between May 2001 and August 2003 were evaluated retrospectively .They had cataract surgery or trabeculectomy combined with cataract surgery at least in one eye. Cases were divided into 3 groups according to their anterior segment findings: Group I patients with PES, group II "preclinic or masked" PES cases, group III the control group. These three groups were compared concerning the age, sex, systemic diseases, anterior segment findings and most particularly their surgical complications. Results: There was no significant difference between PES and suspected-preclinic PES cases according to the age, sex, systemic diseases, visual acuity, ocular tension and the type of ca-taract.There was no statistically significant difference concerning posterior capsuler rupture and zonular dialysis between group I and group II (p0,05). Posterior capsule rupture rate was similar in all groups. Conclusion: In PES cases, cataract surgery is still exposed to many complications and difficulties. "Masked or preclinic pseudoexfoliation criterias" must be well recognized and they may constitute a valuable guide in preventing surgical complications even if there is no pseudo-exfoliation material on their anterior segment.
