Postoperatif endoftalmide yüksek hızda çalışan vitrektör ile total pars plana vitrektomi

Özet
Amaç: Katarakt cerrahisi sonrası endoftalmi gelişen olgularda, yüksek hızda çalışan vitrektör ile total pars plana vitrektomi (PPV) ve intravitreal antibiyotiklerin etkinliğini araştırmak.Gereç ve Yöntem: Bu prospektif çalışmada, katarakt cerrahisi sonrası endoftalmi gelişen 11 olgu incelendi. Endoftalmi geliştikten sonra olguların 3'iine aynı günde, 3'üne 1. günde, 3'üne 1. haftada ve 2'sine de 1. ayda total PPV ve intravitreal vankomisin ve amikasin enjeksiyonu uygulandı. PPV, 1500 kesi/dakika hızda yapıldı. Ayrıca intravitreal antibiyotikler normal kullanılan dozun 1/4'ü kadar uygulandı. Bulgular: Yaşları 35 ile 75 (Ort.: 65) arasında değişen hastalar ortalama 6.6 ay (1-24 ay) takip edildiler. Tüm olgularda preoperatif görme keskinliği ışık hissi düzeyinde idi. Postoperatif görme keskinliği 6 olguda (%55) 1/10 ile 8/10 arasında değişmekteydi ve PPV, bu olguların 3'ünde endoftalminin geliştiği gün, 3'ünde de 1 gün sonra yapılmıştı. Diğer 5 olguya (%45) ise postoperatif dönemde eviserasyon uygulandı. Bu olgulara endoftalmi gelişiminden 1 hafta ile 1 ay sonra PPV uygulanmıştı. Sonuç: Katarakt cerrahisi sonrası endoftalmi gelişen olgularda, erken dönemde, yüksek hızda çalışan vitrektör ile total PPV yapılması ve intravitreal antibiyotiklerin uygulanmasının etkin ve daha emniyetli olduğu gözlendi.
Abstract
Purpose: To evaluate the effectiveness of total pars plana vitrectomy (PPV) with the use of high speed vitrector and intravitreal antibiotics for postoperative endophthalmitis after cataract surgery. Method: In this prospective study, 11 eyes with postoperative endophthalmitis after cataract surgery were evaluated. Total PPV and intravitreal injection of vankomycin and amikacin were performed in 3 eyes at the same day, in 3 eyes at the 1st day, in 3 eyes at the 1 st week and in 2 eyes at the 1st month that endophthalmitis occured. PPV was performed with a speed of 1500 cut per minute. Intravitreal antibiotics were injected as 1/4 of standard doses. Results: Mean age of the patients was 65 years (35-75 years), mean follow-up period was 6.6 months (1-24 months). Preoperative visual acuity was light perception in all patients. Postoperative visual acuity was between 1/10 and 8/10 in 6 eyes (55%); 3 of these 6 eyes had PPV at the same day and the other 3 had PPV at the 1st day after endophthalmitis had occured. Five eyes, which had PPV between 1st week and 1st month after development of endophthalmitis, underwent evisseration. Conclusion: Total PPV with the use of high speed vitrector and intravitreal antibiotics for postoperative endophthalmitis after cataract surgery was observed to be an effective procedure when it was performed in early period.
