Özet

Amaç: Kronik dakriyosistit olgularında dakriyosistorinostominin (DSR) konjonktival flora üzerine etkisini araştırmak.Yöntem: Tek taraflı kronik dakriyosistit olgularında preoperatif konjonktival kültürler alınarak mikroorganizmalar saptandı. Sonuçlar postoperatif 3. aydaki ve sağlıklı diğer gözlerin konjonktival kültür örnekleri ile karşılaştırıldı. Bulgular: Yaş ortalaması 40.8 ± 9.2 yıl (sınırlar, 25-59 yıl) olan 50 olgunun 42'si (%84) kadın, 8'i (%16) erkek idi. Preoperatif olarak dakriyosistitli gözlerin %70'inde, postoperatif olarak %34"ünde üreme izlendi (p=0.001). Kontrol grubu olarak olguların sağlam gözlerinde dakriyosistitli gözlere göre daha az (%48) üreme saptandı (p=0.041). DSR öncesi ve sonrasında çalışma grubunda en çok izole edilen etken Stafilokokus epidermidis (sırasıyla %34, %14) oldu. Kontrol grubunda en çok izole edilen etken olarak Streptokokus pnömonia (%20) tespit edildi. Operasyon öncesinde %12 oranında gram-negatif etken izole edilirken operasyon sonrasında %2 olarak saptandı (p=0.112). Sonuç: Kronik dakriyosistit olgularında DSR operasyonu sonrasında konjonktival floradaki mikroorganizmalarda azalma izlenmektedir.

Abstract

Purpose: To detect the postoperative effect of dacryocystorhinostomy (DCR) operation on conjunctival flora in patients with chronic dacryocystitis. Methods: Conjunctival cultures were taken preoperatively to detect the microorganisms in patients with unilateraly chronic dacryocystitis. The results were compared with the samples of the conjunctival cultures taken postoperatively in the third month and of the other healthy eyes. Results: The mean age of the patients was 40.8 ± 9.2 years (range, 25-59 years), of 50 patients 42 were female (84%), 8 were male (16%). Growth of microorganisms were noted on conjunctival cultures in 70% of cases preoperatively and in 34% of cases postoperatively in eyes with dacryocystitis (p=0.001). In the healthy eyes of patients as the control group, the conjunctival cultures were less positive (48%) than the involved eyes (p=0.041). The mostly isolated microorganism was Staphylococcus epidermidis in eyes with study group both preoperatively and postoperatively (respectively 34%, 14%). On the other hand in the control group, the mostly isolated microorganism was Streptococcus pneumoniae (20%). Preoperatively while 12% gram-negative agents were isolated, postoperatively it was detected 2% (p=0.112). Conclusion: Reduction in yield of microorganisms in conjunctival culture is observed following DCR operation in patient with chronic dacryocystitis.
