Katarakt cerrahisi sırasında vitreus kaybının postoperatif bulgulara etkisi

Effects of vitreous loss during cataract surgery on postoperative findings

Özet

Amaç: Cerrahi esnasında arka kapsül rüptürü gelişip vitreus kaybı olan ve olmayan olguları, arka kapsül rüptürü gelişmeyen olgularla postoperatif görme keskinliği ve postoperatif komplikasyonlar açısından karşılaştırmak. Yöntem: Kliniğimizde katarakt cerrahisi esnasında arka kapsül rüptürü olup vitreus kaybı gözlenen 37 göz 1.Grubu, arka kapsül rüptürü olup vitreus kaybı gözlenmeyen 21 göz 2.Grubu ve arka kapsül rüptürü gözlenmeyen 708 gözden rastgele seçilmiş 68 göz kontrol grubu olan 3.Grubu oluşturdu. Gruplar postoperatif düzeltilmiş görme keskinliği ve komplikasyonlar açısından Ki-kare testi, Kruskal-Wallis testi, Mann-Withney U testi ve Wilcoxon signed ranks testi ile karşılaştırıldı. Bulgular: Her üç grupta postoperatif düzeltilmiş görme keskinliğinde preoperatif görme keskinliğine göre istatistiksel anlamlı artış olmuştur (Wilcoxon signed ranks test, p:0,0001). Her üç grubun postoperatif görme keskinliği açısından yapılan ikili karşılaştırmasında 2. ve 3.grup arasında istatistiksel anlamlı fark yok iken 1. ve 2. ile 1. ve 3.grup arasında anlamlı fark saptandı(Kruskal-Wallis test, p:0,0001). Pupil çekintisi, yeni başlangıçlı sekonder glokom, kistoid makuler ödem, retina dekolmanı gibi vitreus kaybına bağlı postoperatif komplikasyonlar grup İve 2' de daha sık iken arka kapsül kesifleşmesi grup 3' te daha sıktır. Sonuç: Katarakt cerrahisi vitreus kaybı olsun yada olmasın preoperatif görme keskinliğine göre postoperatif düzeltilmiş görme keskinliğinde artışa yol açar. Ancak bu artış vitreus kaybı olan olgularda daha düşüktür.

Abstract

To compare cases with posterior capsule rupture with and without vitreous loss and cases with intact posterior capsule during cataract surgery by means of postoperative visual acuity and complications. Methods: Patients are divided into 3 groups. Group 1 included 37 eyes that had posterior capsule rupture with vitreous loss, Group 2 included 21 eyes that had posterior capsule rupture without vitreous loss and Group 3 included 68 eyes selected randomly from 708 eyes that had not posterior capsule rupture during cataract surgery in this study. Postoperative visual acuity and complications observed in these 3 groups were compared with Ki-square test, Kruskal-Wal-lis test, Mann-Withney U test, Wilcoxon signed ranks test. Results: There was statistically significant increase in postoperative visual acuity compa¬red with preoperative state in these three groups (Wilcoxon signed ranks test, p: 0,0001). 3 groups were compared within each other: there was no statistically significant difference between Group 2 and 3, statistically significant difference was found between Group 1 and 2 and Group 1 and 3 concerning postoperative visual acuity (Kruskal-Wallis test,p: 0,0001). Incidence of postoperative complications such as pupillary distortion by vitreous band, newly detected secondary glaucoma, cystoid macular edema,retinal detachment were higher in group 1 and 2, however posterior capsule opacification was observed higher in Group 3. Conclusions: Cataract surgery with or without vitreous loss increases postoperative corrected visual acuity when compared to preoperative visual acuity values. But this increment is less than cases with vitreous loss.
