Trabekülektomili olgularda temporal yaklaşımlı fakoemulsifikasyonun göz içi basıncı ve görme keskinliği üzerine etkisi

Özet
Amaç: Daha önce trabekülektomi (TE) uygulanmış ve fonksiyonel bir blebi olan gözlerde, temporal korneal yaklaşımlı fakoemülsifikasyonun (FE) jgöz içi basıncı (GİB), glokom ilacı sa¬yısı, blep fonksiyonları ve görme keskinliği üzerindeki etkilerinin araştırılması amaçlandı. Materyal ve Metod: Temporal yaklaşımlı FE uygulanan, daha önce TE geçirmiş 22 olgu¬nun 23 gözüne ait kayıtlar geriye dönük olarak incelendi. Onbeş göz primer açık açılı glokom (PAAG), 8 göz psödoeksfolyatif glokomdu (PEG). Hastaların yaş ortalamaları 64.5±5.9 (55-78 yıl) yıldı. FE'nun TE'den ortalama 38.3±20.7 ay (10-48 ay) sonra gerçekleştirildiği saptandı. FE sonrası ortalama 21.2±10.7 ay (10-45 ay) süreyle takip edilen olguların 1, 3, 6, 9 ve 12. aylara ait bulguları kaydedildi (GİB, görme keskinliği, glokom ilaç sayısı, bleb görünümü). İstatistik¬sel değerlendirmelerde paired t test, Wilcoxon signed rank test ve ki kare testi kullanıldı. İstatis¬tiksel anlamlılık p0.05). Sonuç: TE'li gözlerde katarakt ameliyatı gündeme-geldiğinde, temporal korneal yaklaşımlı FE'nun gerek GİB kontrolü, gerekse blep fonksiyonlarının korunmasında, iyi bir cerrahi seçe¬nek olduğunu düşünmekte ve önermekteyiz.
Abstract
Purpose: To evaluate the effect of temporal clear corneal phacoemulsification on the intra¬ocular pressure (IOP), antiglaucomatous medication, bleb function and visual acuity in eyes with previous trabeculectomy. Materials and Methods: Temporal clear corneal phacoemulsification was performed in 23 eyes of 22 glaucoma patients and medical records of the patients were evaluated retrospecti¬vely. Fifteen eyes had primary open angle glaucoma and 8 eyes pseudoexfoliative glaucoma. Mean age of the study group was 64.5±5.9 (55-78) years. The mean interval between trabecu¬lectomy and phacoemulsification was 38.3±20.7 (10-48) months. Mean postoperative follow-up period was 21.2± 10.7 (10-45) months and medical records of the patients 1, 3, 6, 9 and 12 months were evaluated (IOP, visual acuity, the number of antiglaucomatous medications, bleb appearance). For statistical analysis, paired t test, Wilcoxon signed rank test and chi-square test were used. Results: The mean preoperative IOP was 15.2±4.1 mmHg (8-22 mmHg) and the mean postoperative IOPs were 14.8±3.5 mmHg, 15.4+2.9 mmHg, 15.813.1 mmHg, 15.7±2.8 mmHg and 16.2±2.2 mmHg at 1, 3, 6, 9 and 12 months respectively. Mean number of antiglaucoma medication were 0.48±0.6 (0-2) preoperatively, while it was 0.40±0.5, 0.43±0,6, 0.48±0.6, 0.48±0.6, 0.52±0.6 at 1, 3, 6, 9 and 12 months respectively. The patient that not need antiglau¬comatous medication was 60.8% before phacoemulsification and, this ratio decreased to 52.2% after phacoemulsification. The filtering blebs were functional in all patients and no patient nee¬ded additional glaucoma surgery after phacoemulsification. The increase in IOP and the number of antiglaucomatous medications used was not statistically significant before and after phacoe¬mulsification at any follow-up examinations (p>0.05). Conclusion: Temporal clear corneal phacoemulsification is a safe and efficious choice in the surgical management of cataracts in eyes that underwent prior trabeculectomy.
