Penetran keratoplasti sonrası kornea topografisinde longitudinal değişimler

Özet
Amaç: Penetran keratoplasti sonrası kornea topograf isinde longitudinal değişimleri saptamak. Yöntem: Çalışmamıza penetran keratoplasti geçirmiş 17 hastanın 17 gözü dahil edildi. Hastaların yaşı, cinsiyeti, keratoplasti endikasyonu, oto-keratometri değerleri manifest refraksiyon değerleri ve sütür tipi kaydedildi. Postoperatuar 1. hafta, 1. ay, 3. ay ve 6. ayda bilgisayarlı kornea topografisi uygulandı. Sim K değerleri, korneal silendir değerleri, potansiyel korneal keskinlik ("potential corneal acuity", PCA), yüzey regülerite indeksi ("surface regularity index", SRI) ve yüzey asimetri indeksi ("surface asymmetry index", SAI) değerleri kaydedildi. Vektöriyel analiz ile postoperatuar 1. hafta başlangıç değerine göre 6. aydaki indüklenen astigmatizma miktarı ve sferik eşdeğer değişikliği hesaplandı. Kornea topografisi desenleri Karabatsas sınıfla¬masına göre sınıflandı. Bulgular: Ortalama Sim K değerleri takip boyunca stabildi (p=.348). SAI değerlerinde 3. ve 6. aylar arasındaki fark istatistiksel olarak anlamlıydı (p=.008). Takip boyunca ortalama si¬lendir, ortalama SRİ ve ortalama PCA değerlerinde değişiklik izlenmedi (sırasıyla p=.272, p=.272 ve p=.682). Altıncı ay sonunda 1. haftaya nazaran zamanın indüklediği astigmatizmanm aritmetik ortalaması 3.65±0.71; skalar ortalaması 1.64®32 idi. Aynı dönemler arasındaki sferik eşdeğer değişikliği -1.35±1.34 (-9.82- +3.68) idi, diğer bir deyişle hipermetropi yönünde bir değişiklik mevcuttu. Topografik desenler incelendiğinde operasyon sonrası 1. haftada 8 olguda (%47.1), 1. ayda 11 olguda (%64.7), 3. ayda 9 olguda (%52.9) ve 6. ayda 8 olguda (%47.1) regüler desenler mevcuttu. Tartışma: Penetran keratoplasti sonrası ilk 6 ay boyunca korneada düzleşme mevcuttur, Altıncı ayda regüler ve irregüler topografi desenlerinin dağılımı yaklaşık eşit orandadır.
Abstract
Purpose: To determine the longitudinal changes in corneal topography after penetrating keratoplasty. Material and method: Seventeen eyes of 17 patients who underwent penetrating keratop¬lasty were involved in this study. The information reviewed included age, sex, indication for ke¬ratoplasty, auto-keratometry measurements, manifest refraction, and suturing technique. Com¬puterized corneal topographies i of the patients postoperatively at 1 week, 1 month, 3 months, and 6 months were performed. Sim K values, corneal cylinder, potential corneal acuity (PCA), surface regularity index (SRI), and surface asymmetry index (SAI) were recorded. The induced astigmatism and spherical equivalent change from 1 week to 6 months postoperatively were cal¬culated by vector analysis. Trie topographical patterns were classified according to the Karabat-sas classification. Findings: The mean Sim K values were stable over the follow-up period (p=.348). There was a statistically significant difference between the mean SAI at 3- and 6- months (p=.008). During the follow-up, the mean cylinder, SRI, and PCA values were stable (p=.272, p=.272, and p=.682 respectively). From 1 week to 6 months postoperatively, the arithmetical mean of the induced astigmatism was 3.65±0.71; and the scalar mean was 1.64@32. During the same ti¬me period, the spherical equivalent change was -1.35±1.34 (-9.82-+3.Ö8); i.e. there was a hype-ropic shift. Regular topographic patterns were seen at 8 cases (47.1%) at postoperative week 1, 11 cases (64.7%) at postoperative month 1, 9 cases (52.9^) at postoperative month 3, and 8 ca¬ses (47.1%) at postoperative month 6. Discussion: There is corneal flattening in the first 6 months after penetrating keratoplasty. At 6 months postoperatively, the distribution of regular and irregular astigmatic patterns are al¬most equal.
